About this file
This is a case scenario document from Families Together Building Solutions (FTBS), a stepdown service provider working in conjunction with child protective services (CPS). The document outlines a referral for continued family support services following the successful completion of Families First Mentoring (FFM) services. The case involves a single mother, Ms. Doe, and her two young children (ages 4 and 3) who were subjects of a CPS investigation involving physical abuse and improper supervision. The referring worker has requested FTBS to provide ongoing services focused on increasing positive parenting techniques, ensuring appropriate child supervision, identifying both formal and informal community supports, facilitating a substance use evaluation for Ms. Doe, and providing education on healthy relationships and community resources.
The case presents a family with demonstrated capacity for improvement, evidenced by their successful completion of FFM services and risk reduction. Ms. Doe's strengths include gainful employment, financial resources, and receptiveness to services; however, challenges include inconsistent work, limited social support systems, and identified needs related to substance use and domestic violence history. The family has adequate housing and Ms. Doe has expressed acknowledgment of needing assistance, creating a foundation for continued service engagement. No specific pricing terms, contract timelines, or funding mechanisms are detailed in this scenario document, which appears to function as a case intake and service planning reference rather than a formal procurement specification.

Text version
Jane Doe was referred to Families Together Building Solutions as a stepdown service. The family successfully completed FFM services.
The CPS case was dispositioned as a Category II due to physical abuse and improper supervision. The children, Mary (age 4) and Bobby (age 3), were found wandering unsupervised two blocks from home, the police were called resulting in a report to CPS. Bruises were found on the children and Ms. Doe admitted to using physical discipline. Ms. Doe’s boyfriend recently moved out of the family home; Ms. Doe reported a history of domestic violence and substance use with her boyfriend.
The family made improvements throughout the FFM intervention as education regarding appropriate parenting/discipline, domestic violence and substance use (including the effects on children) were provided. The family followed agreed upon safety plans to improve supervision and increase child safety. As a result, the risk was reduced at the time of FFM case closure.
The Referring Worker requested FTBS to continue focusing on increasing positive parenting techniques and ensuring ongoing appropriate supervision, to include identifying appropriate formal and informal supports. Education of community resources was also identified as a need at the time of case referral. The RW requested the FTBS worker assist Ms. Doe in seeking an evaluation to address her substance use. Further education regarding healthy relationships was also requested.
Ms. Doe is gainfully employed, though her work is not consistent. The family has adequate housing, but a limited social support system. Her strengths include financial resources, acknowledgment of needing assistance and receptiveness to services. Ms. Doe also identified reduced stress as a result of her ex-boyfriend moving out of her home.
