About this file
This is Amendment 1 to RFP 210000001912 issued by the Michigan Department of Health and Human Services Bureau of Grants and Purchasing for the Families Together Building Solutions program. The amendment modifies contractor responsibilities and service delivery requirements, specifically defining position-specific requirements for Program Managers, Supervisors, and FTBS Direct Workers. Key responsibilities include review and approval of service plans, conducting bi-weekly individual supervision, facilitating weekly team meetings, accompanying staff on home visits, and ensuring completion of required training and data reporting. The estimated volume of service is 120 eligible clients over the contract period (40 per year), with services measured in units where one unit equals one hour of direct contractor time including face-to-face contact, collateral and telephone contacts, travel time, training time, and case-related activities.
The amendment establishes the unit of service definition and measurement framework for billing and payment purposes under this unit rate contract. Contractors must submit proposed units on the Schedule B Pricing Matrix. A Training Schedule (Schedule C) is attached to the RFP. The contractor is responsible for ensuring all staff attend required FTBS training within specified timeframes and that all required data is accurately reported to the MDHHS Local Office and Family Preservation Program Office on time. Additional contract performance requirements include monthly case reviews, new worker orientation and training, and maintenance of 24/7 service availability through adequate staffing and back-up coverage.
