Schedule A - Statement of Work Summary

This is a Statement of Work document issued by the Michigan Department of Transportation (MDOT), Bureau of Field Services, Fleet Division for Request for Proposal (RFP) 220000000559 regarding Hydraulic Hoses, Fittings & Components. MDOT seeks to establish up to three optional-use contracts for the purchase of hydraulic hoses, fittings, and components used for buildup and repair of MDOT vehicles and equipment across seven Regional Service Areas. The products must be the latest models in current production and must comply with American National Standards Institute (ANSI), Occupational Safety and Health Administration (OSHA/MIOSHA), Society of Automotive Engineers (SAE), Federal Motor Vehicle Safety Standards (FMVSS), and applicable State of Michigan standards and regulations. Core parts are listed in Schedule B - Pricing, with reference brands including Tompkins and Parker Hannifin. The RFP document does not specify explicit response deadlines, bid submission dates, or contract award dates within this excerpt, though it references separate Proposal Instructions for the Question and Answer period timing.

Pricing is firm for 365-day periods beginning on the contract effective date, with adjustments based on documented changes in actual contractor costs and supported by evidence such as Consumer Price Index data, Producer Price Index, or manufacturer letters. The contractor must provide manufacturers' product literature supporting performance and design specifications, offer their entire product line at a percentage discount off manufacturer suggested retail price (MSRP), and provide portable and bench-mounted crimpers at no cost to MDOT upon request. Service level requirements include delivery of regular orders within 48 hours and emergency orders within 24 hours of receipt, with deliveries accepted Monday through Friday between 8:00 a.m. and 3:00 p.m. at designated MDOT garage facilities. The contractor must provide a one-year warranty or manufacturer's warranty (whichever is greater), quality assurance programs, free operator training on crimper equipment, technical support availability from 8:00 a.m. to 5:00 p.m. Eastern Time, and immediate notification and assistance with defective part removal and replacement at no cost. Liquidated damages of 10 percent of applicable invoices apply for failure to meet specifications or delivery deadlines if not remedied within five business days. Payment will be made exclusively through Electronic Funds Transfer (EFT) as required by Michigan law.
