Job Hazard Analysis

PRINCIPAL STEPS POTENTIAL HAZARDS RECOMMENDED CONTROLS
This column is used for the different steps used for the Principal Activity
This column is used for Potential Hazards that go hand in hand with each Principal Step
This column is used to the recommended controls that go with the Principle Steps and Potential Hazards
Remember each Activity should have multiple steps in order to accomplish the task
Each Step can have multiple hazards One Principle Step can have multiple Potential Hazards which means you need to have at least one Recommended Control for each Potential Hazard
You are probably asking how many steps are needed…. That is determined by the complexity of the Activity…
1. Make sure that you bullet out each hazard so you can line them up with the Recommended Controls
2. It is so much easier to read and understand the finished product when they are bulleted
1a. Keep the bullets in order of the Potential Hazards for example if you have more than one recommended control you could use
1b. Multiple bullets 2a. Then move onto the next control for the following hazard
Do not put so much information in this document that it overwhelms the employees
Again do not put so much information in this column that it overwhelms the employees
Preferably we would use the Hierarchy of Controls to keep the employees safe but many times PPE is the only answer;
a) Engineering (Engineer the hazard out)
b) Administrative (Training, Signage, OJT etc.)
c) PPE (Make sure that the PPE covers the hazard i.e. leather gloves do not cover all hand hazards) Do put enough information that anyone can understand what the principle activity is
You still need to ensure that it has enough information for everyone to understand
This is still basic information but you can add a bit more meat to the context… for example if you just say the employee needs to wear gloves the type needs to be present… Do they need Leather, Chemical Resistant, Cold Weather gloves???

Mobilization
1. Driving

3. Slips Trip & Falls
4. Lifting
5. Cut, Scratches and Abrasions
1a. Utilize Defensive Driving Techniques, 1b. Get Out And Look (G.O.A.L.)
2a. Watch for proper body placement 3a. Situational Awareness, eyes on path 3b. Housekeeping 4a. Utilize proper lifting techniques 4b. Use mechanical or buddy lifts for heavy and or awkward lifts 5a. Situational Awareness watch hand placement 5b. Leather gloves
TOOLS AND EQUIPMENT
TO BE USED INSPECTION REQUIRED TRAINING REQUIRED
Examples:
Laser
Laser is to be checked daily against a plumb point to verify calibration All individuals using a laser are to be certified in its use and carrying a certification card.
All individuals employed with Phase 2 company receive training on limiting exposure to laser radiation.
Scissors lift
All scissors lifts on site will be inspected daily by the user and the designated competent person. Inspection will be certified by signing and dating an affixed scaffold tag on all machines controlled by Phase 2 Company
All individuals utilizing scissors lifts will be certified in the use of the specific type of lift being operated.
All individuals will have a certification card on their person.
PAT (powder actuated tool) All powder actuated tools will be checked in and out with the site foreman or competent person daily. Tools will be field stripped and cleaned daily, inspected for damage or wear then reassembled.
All employees utilizing Powder actuated tools will have received basic instruction and been certified in their use within the last three years and have within their possession a wallet card provided by Phase 2 Company.

JOB HAZARD ANAYLISYS WILL BE DISSCUSSED AND SIGNED BY ALL PARTICIPANTS PRIOR TO WORK ACTIVITY.
If there is a need to deviate from planned safe work practices or procedures, work must be stopped and discussed with appropriate personnel

When someone signs a document they need to take ownership of it…
I also find that if we get the everyday employee, foreman or supervisors help in writing the JHA’s they tend to be more accurate and they are more willing to comply
