Attachment 02 performance work statement for:
Anderson Ditch Maintenance National Institute of Standards & Technology United States Department of Commerce
325 Broadway Boulder, CO 80305 August 18, 2016
DESCRIPTION
Provide all labor, equipment, and materials to clean the hard deck section of the Anderson Irrigation Ditch to comply with current standards on the National Institute of Standards and Technology (NIST) site cited here in this document. The work is to be performed in accordance with this Performance Work Statement (PWS).
PURPOSE
The purpose of this project is to clean the hard deck section of the Anderson Irrigation Ditch. The work will consist of the following:
· Site work
The work is described in herein section 1.4, “Summary of the Work” and shall be code compliant as described in section 1.5 “Reference Standards,” safe, accessible, and easily maintainable. This PWS is written in a manner that allows the quoter to prepare a price quotation for each of the areas of work described in this document independent from each other.
· This requirement will result in an award of a Firm Fixed Price (FFP) purchase order with a base year plus four (4) one-year-options.
The contractor shall:
· Perform the work as described in this PWS;
· Perform a site walk to fully understand the work required and the local conditions;
· Prepare and follow a site specific safety plan to be reviewed and accepted by NIST Safety Staff;
· Perform to the daily Job Hazards Analysis (JHA) for required service activities;
· Perform all utility locates; and
· Coordinate all work with the Contracting Officer’s Representative (COR).
SPECIAL REQUIREMENTS
· Pre-quotation site visit is required.
· Kickoff meeting is required.
· Method of work shall be submitted with the quotation, including a description of how the contractor intends to protect surrounding areas.
Summary of the Work General The contractor shall:
Provide all labor, furnish and install materials, equipment and supplies in accordance with the Performance Work Statement.
Provide with the quotation a Delivery Schedule for which each service shall be completed within the time frame stated in section B of this document for each year. The contractor shall provide the detailed project schedule in Microsoft Project with the quotation. Final schedule shall be agreed upon after award during the kickoff meeting. The kickoff meeting shall be held 10 days after award is given.
Provide project management and field supervision throughout the project duration.
Be familiar with the work required and local conditions affecting the work.
Ensure the safety of the project is a top priority. Submit a site specific safety plan with project with the quotation.
Ensure due diligence and timely delivery of the project.
Correct errors and deficiencies for the services provided.
Adhere to NIST Policy when accessing the site and maintaining site security. Ensure contractor personnel meet all site access requirements. Provide a list of all anticipated employees with the quotation. Specified background check forms shall be completed after award for site access is authorized and before any security badges are issued. The forms shall be complete and typed.
Maintain a record of all inspections, or other authorizations necessary for the work to proceed.
Notify the COR twenty-four (24) hours in advance of all required inspections and as work progresses.
Correct any damage caused to the NIST Boulder site and field sites as a result of their work and return the site to previous conditions at no cost to the Government.
The Contractor shall perform final cleaning and detailing of work areas.
Contractor is responsible for determining how work shall be accomplished in the most advantageous manner to the Government.
Notify the COR regarding any conflicts associated with this Performance Work Statement, drawings and any appendixes.
Demobilize all elements of the Project.
Specification Section Site work includes the following:
The hard deck section of the Anderson Ditch from the northern most terminus of ditch to the terminus of the hard deck underground section south of Rayleigh Road per the map included in Attachment 2a shall be clean and free of debris.
Hard deck section includes the following (all measurements shall be verified by the contractor prior to quotation):
One Concrete Box culvert approximately 90 feet in length and 10 feet wide.
One Concrete Box Culvert approximately 340 feet in length and 10 feet wide.
Open section of concrete ditch approximately 720 feet in length and 8 feet wide.
Standards Hard deck section floor currently consists of cobblestone embedded in concrete.
Bottom of concrete ditch shall be free of sediment. No more than .5” of silt shall remain after clean operations to be considered free of sediment.
All vegetation and debris within the limits of the concrete ditch shall be removed.
Care must be taken not to damage the bottom lining of the ditch, surrounding landscape, and concrete sidewalk. The Contractor is responsible for repairing any damage caused by operations.
All sediment and debris removed from limits of ditch shall be disposed of offsite.
Box Culverts shall consist of smooth concrete floor.
Bottom of box culvert shall be free of sediment. No more than .5” of silt shall remain after cleaning operations to be considered free of sediment.
Care must be taken not to damage surrounding landscaping and concrete sidewalk. The Contractor is responsible for repairing any damage caused by operations.
All sediment and debris removed from the box culverts shall be disposed of offsite.
Video Inspection of the box culvert cleanings is required, and shall be submitted to the COR.
Sediment and debris shall not be washed passed the end of the hard deck section at Rayleigh road.
Submit method of cleaning with the quotation, which shall be approved by the COR prior to mobilization.
NOTE: The concrete ditch cannot be accessed from May 1st through October 1st each contract year.
The two detention ponds on the NIST property per the map included in Attachment 2a shall be clean and free of debris.
Detention ponds include the following:
North Parking lot detention pond consists of one concrete channel and one trash grate.
South Parking lot detention pond consists of two concrete channels, two side by side 12” culverts and two trash grates. Total channel dimensions are approximately 55 feet in length and 12 inches wide. Two 12” culverts are approximately 20 feet in length each.
Standards The concrete channels and trash grates shall be free of sediment and debris.
The culverts shall be free of sediment.
Care must be taken not to damage the surrounding landscape. The Contractor is responsible for repairing any damage caused by operations.
All sediment and debris removed shall be disposed of offsite.
Base year cleaning shall take place in the Fall of 2016. All option year operations for the performance work statement shall be performed a minimum of once per contract year between the end of fall and beginning of spring season during the access dates identified herein.
Removal is based on normal site conditions. Unusual build up due to flooding shall be considered and scheduled appropriately for each contract option year.
