About this file
This document is a sample change order template for Pinellas County Purchasing Department in Clearwater, Florida. The form is designed to modify existing contracts by documenting additions or deletions to the original agreement amount, with fields for fiscal number, change order number, account number, total agreement amount, addition amount, deletion amount, and new total agreement amount. The template does not specify particular products, services, quantities, dates, or contract terms as it serves as a standardized form for use across various procurement activities.
