About this file
This document is a Sample Change Order template from the Pinellas County Purchasing Department in Clearwater, Florida, designed to facilitate modifications to an existing contract. The template provides a standardized format for documenting changes to total agreement value, including potential additions or deletions, with spaces to record fiscal number, change order number, and account number. The form includes provisions that require the contractor to acknowledge that executing the change order constitutes a waiver of additional time or compensation claims related to the changes, and that the authorized payment represents full compensation for all associated work.
The change order template includes signature lines for multiple parties, including an Investigator, Director of Purchasing, County Attorney, Board of County Commissioners Chairman, County Clerk, and the contractor, ensuring multiple levels of review and approval. While this specific document is a blank template, it is associated with a Pinellas County Public Works project for Cured-In-Place Pipe (CIPP) Stormwater Pipe Rehabilitation, which aims to rehabilitate the county's stormwater and sanitary sewer infrastructure. The change order process allows for flexible contract management, enabling adjustments to scope, cost, and terms while maintaining a formal, documented approach to contract modifications.
