Project Title: Replace CLC3 Finishes Project Location: Lake City Veterans Affairs Medical Center Lake City, FL 32608 IAW VAAR 836.204 the magnitude of construction VA project is: between $250,000.00 and $500,000.00 NAICS: 236220 - Commercial and Institutional Building Construction Service Code: Y1DA - Construction of Hospitals and Infirmaries Project Description: This project will provide design and construction services for the replacement of CLC3 finishes at the Lake City VAMC, Lake City, FL. All work is to be completed in accordance with the requirements.
This solicitation will be evaluated on a Tradeoff basis. As stated elsewhere in this solicitation, this procurement is a Two-Phase Design-Build IAW FAR 36.3.
One solicitation will be issued covering both phases. In Phase One proposals will be evaluated to determine which offerors will submit proposals for Phase Two. Block 13a, refers to Phase One proposals.
THIS ACQUISITION IS SET-ASIDE 100% FOR SERVICE-DISABLED VETERAN-OWNED SMALL BUSINESS (SDVOSB) ONLY, UNDER THE AUTHORITY OF 38 U.S.C. PART 74. ANY PERSON(S) OR BUSINESS ENTITY SUSPECTED OF MISREPRESENTATION FOR THE PURPOSE OF THE SECURING A GOVERNMENT CONTRACT MAY BE CRIMINALLY INVESTIGATED AND PROSECUTED FOR FRAUD AGAINST THE UNITED STATES GOVERNMENT.

INSTRUCTIONS TO OFFERORS AND EVALUATION INFORMATION
I. GENERAL PROPOSAL INFORMATION
1. Solicitation, attachments, and any issued amendments will be posted at www.sam.gov. This is the only official location where these documents can be obtained and where updates are posted. No “hard copy” solicitation materials are available.
2. All references to “Bid” or “Bidder” shall be understood to mean “Offer” or “Offeror.”
3. MAGNITUDE OF THIS PROJECT: Between $250,000.00 and $500,000.00
4. PROPOSAL DUE DATE: Standard Form (SF) 1442, block 13a (or as amended on SF 30), refers to Phase One proposals.
5. SITE VISIT: No pre-proposal/Site Visit will be conducted during Phase One of this solicitation.
Those offerors selected to participate in Phase Two of the solicitation will receive information about Phase Two pre-proposal conference/Site Visit in the invitation to participate in Phase Two.
6. OFFER ACCEPTANCE PERIOD: Offers providing less than one-hundred and twenty (120) calendar days for Government acceptance after the date offers are due will not be considered and will be rejected.
7. REQUESTS FOR INFORMATION: Requests for Information (RFIs) shall be submitted in writing (only) by the Prime Contractor (SDVOSB) to the Contracting Officer’s email at calixte.saintil@va.gov. No telephone RFIs/Questions will be accepted. The cutoff for submission of RFIs for PHASE ONE is 06/26/2023 @ 3:30PM (EST). Responses will be provided in a Question & Answer format to all offerors via Standard Form (SF) 30 Amendment to the Solicitation.
RFI received after due date will only be answered if in the judgement of the CO impacts the project/ solicitation.
8. SPECIFICATIONS AND DRAWINGS: All applicable specifications and drawings will be provided electronically along with the solicitation. No hard copies will be made available to offerors.
9. Any reference contained within contract specifications and/or drawings to the “VA Engineer,” “Resident Engineer,” “Senior Resident Engineer,” “Project Manager,” “Contracting Officer Technical Representative,” or their abbreviations are to be replaced with “Contracting Officer Representative (COR)”
10. SERVICE-DISABLED VETERAN-OWNED SMALL BUSINESS VERIFICATION: This solicitation is 100% set asides for verified Service-Disabled Veteran-Owned Small Businesses (SDVOSB). The award of this requirement shall not be delayed due to loss of SDVOSB verification.
Proposals submitted by non-verified SDVOSB shall be excluded from award consideration.
11. SDVOSB Verification: The Government will review SBA VetCert (https://veterans.certify.sba.gov) to confirm SDVOSB status verification for the submitted proposal. Evaluations will be performed on proposals submitted by verified SDVOSBs only. Subsequent confirmation of SDVOSB verification status will be performed for the apparent awardee.

12. LIMITATIONS ON SUBCONTRACTING (LOS) (Reference Contract Clause FAR 52.219-14 for further information): By submission of an offer and execution of a contract, the Offeror agrees that in performance of the contract, the concern will perform at least fifteen (15) percent of the cost of the contract, not including the cost of materials, with its own employees.

COMPLIANCE FOR SERVICES AND CONSTRUCTION (SEP 2021) (DEVIATION) shall be signed on the SF 1442 and submitted.
13. FUNDS: Funds are not presently available for this acquisition. The Government does not compensate offerors for proposals’ development. Award is subject to availability of funds, in accordance with FAR 52.232-18 Availability of Funds and VAAR 807-173.
14. BONDS: Performance Bonds (SF25) and Payment Bonds (SF25A) are required. Offeror must refer to SF1442 Block 12a for Payment and Performance Bond requirements. Reference Contract Clause FAR 52.228-15 for further information on applicability.
15. OFFER GUARANTEE (BID BOND) (SUBMIT WITH YOUR PHASE TWO PRICE
PROPOSAL): Offeror must refer to SF1442 Block 13B (Bid Bond) requirement. Per FAR Clause 52.228-1-Bid Guarantee, the amount of the bid guarantee shall be 20 percent of the bid price or $3,000,000, whichever is less. NOTE: A legible electronic copy of the original is acceptable;
offerors invited to Phase Two are required to submit bid bonds. The bid bond will be submitted with Phase Two Price Proposal in a separate PDF document (do not combine/save with other documents) and shall be named “BID BOND – OFFEROR COMPANY NAME”.
16. SAFETY AND ENVIRONMENTAL RECORD (SUBMIT WITH PHASE ONE PROPOSAL):
Offerors shall submit information that specifies the contractor in question has no more than three serious, or one repeat, or one willful Occupational Safety and Health Administration (OSHA) or any Environmental Protection Agency (EPA) violation(s) in the past three (3) years and has an Experience Modification Rate (EMR) should not exceed 1.0. If no EMR, state the reason way no EMR is available. EMR greater than 1.0 will not be acceptable. Offerors are required to complete and submit attached (Attachment 09 - Safety or Environmental Violations and Experience Modification Rate EMR Form) Past safety and environmental record or any other form that provided the above information. Offerors with an EMR greater than 1.0 will be considered ineligible and will not be evaluated.
17. VETS-4212, Federal Contractor Veterans’ Employment Report (SUBMIT WITH PHASE ONE PROPOSAL): Offerors are required to provide verification of their submission of the annual form VETS-4212 (https://www.dol.gov/agencies/vets/programs/vets4212). This verification shall be the emailed confirmation of receipt notification sent to the offeror after successful submission of their report.
18. PERFORMANCE PERIOD: Refer to SF1442, Block 11, for performance period. Performance period shall be commenced upon receipt of written Notice to Proceed (NTP) performed in concurrent or sequential phases as indicated or required.

19. E-VERIFY SYSTEM: Companies awarded a contract with the federal government shall enroll in E- Verify within 30 days of the contract award date. They shall also begin using the E-Verify system to confirm that all their new hires and their employees directly working on federal contracts are authorized to legally work in the United States. E-Verify is an Internet-based system that allows an employer, using information reported on an employee's Form I-9, to determine the eligibility of that employee to work in the United States. There is no charge to employers to use E-Verify. (FAR 52.222-54).
20. Section 743(a)(3) of Division C of Public Law 111-117: Companies awarded a contract with the federal government shall comply with the annually reporting requirements of 52.204-14, Service Contract Reporting Requirements and 52.204-15, Service Contract Reporting Requirements for Indefinite-Delivery Contracts. Note that the contracting officer shall make the contractor’s failure to comply with the reporting requirements a part of the contractor’s performance information under FAR Subpart 42.15.
II. PROPOSAL PREPARATION AND EVALUATION INFORMATION
PROPOSAL PREPARATION INSTRUCTION
1. PROPOSAL PREPARATION INSTRUCTION:
1.1. The submission of the documentation specified below will constitute the Offeror’s acceptance of the terms and conditions of the RFP, including attachments and any issued amendments.
1.2. To assure timely and equitable evaluation of proposals, offerors must follow the instructions contained herein.
1.3. In order for proposals to receive full consideration for award, offerors should ensure that the information furnished in support of the proposal is factual, accurate, and complete, and directly responds to the requirements of this solicitation.
1.4. These instructions prescribe the format for the proposal and describe the approach for the development of proposal data. These instructions are designed to ensure the submission of necessary information to provide for the understanding and comprehensive evaluation of proposals.
PROPOSAL SUBMISSION
2. PROPOSAL SUBMISSION:
2.1. All Phase One proposals shall be submitted by the Prime Contractor no later than (NLT) closing date and time specified in SF1442, Block 13.A. (subject to amendment) via email at calixte.saintil@va.gov. NO OTHER FORMS OF SUBMISSION WILL BE ACCEPTED.
2.2. Your proposal shall be submitted as portable data files (pdf), and documents submitted shall be no larger than 7 Mega Bytes (MB) in size. Send document in separate files as deem necessary. The offeror is responsible to verify that the Government has received the proposals.
2.3. Undeliverable emails due to an offeror’s proposal size, using wrong email address, or else, is not the responsibility of the Government.

2.4. To be considered and accepted as timely the proposal must be received in its entirety prior to the deadline. Failure to provide a proposal in its entirety prior to the deadline will render the proposal noncompliant.
BASIS FOR AWARD
3. BASIS FOR AWARD: It is the Government’s intent to award one (1) firm-fixed price contract resulting from this solicitation to the responsible offeror whose offer, conforming to the solicitation, is fair and reasonable, and has been determined to be the most advantageous to the Government, non-price, price, and other factors considered. The proposals must conform to the solicitation requirements and judged to represent the Best Value to the Government. The source selection process will utilize FAR Part 36.3 Two Phase Design-Build Selection Procedures and FAR 15 to evaluate proposals and award the contract.
3.1. All evaluation factors other than price, when combined, are approximately equal to price. As differences in factor ratings and relative advantages and disadvantages become less distinct, difference in price may become the discriminating factor in determining the most advantageous offer. Conversely, as differences in price become less distinct, differences in ratings and relative advantages and disadvantages between offers are determinative.
3.2. Intent for No Discussions: Offerors are hereby advised that the Government intends to evaluate proposals and award a contract without conducting discussions with offerors. However, the Government reserves the right to contact Offerors and/or hold discussions if the Contracting Officer determines them to be necessary. Offerors are cautioned that discussions may not be conducted, and Offerors should not assume that they will have an opportunity to revise their proposals after submission.
SELECTION AND EVALUATION PROCEDURES
4. SELECTION AND EVALUATION PROCEDURES: This part describes the mechanism for evaluating and selecting offers for this two-phase design-build procurement.
4.1. This solicitation addresses both the Phase One and Phase Two proposal requirements.
4.2. The Phase One proposals will be evaluated to determine which offerors will be invited to submit Phase Two proposals. Do not submit Phase Two proposals unless specifically instructed by the Contracting Officer.
4.3. After evaluating Phase One proposals, the Government intends to select a maximum of five (5) of the most highly qualified offerors and those will be invited to submit a Phase Two proposals. However, the Government reserves the right to reject any and/all offers.
4.4. Do not submit pricing with the Phase One proposal. Phase One does NOT involve a pricing submission or evaluation of price.
4.5. In Phase Two, Offerors will compete to determine which Offeror will be awarded the Firm Fixed- Price contract. For Phase Two, Offerors must submit technical and price proposals. The Government reserves the right to reject any and/all offers in Phase Two.
PROPOSAL REQUIREMENTS AND FORMATTING (PHASE ONE AND PHASE TWO)
5. PROPOSAL REQUIREMENTS AND FORMATTING (PHASE ONE AND PHASE TWO):
5.1. Proposals shall be typewritten, with fonts no smaller than 10 points, with page layout no smaller than
½” top, bottom and ½” side margins, single space, paginated, portrait mode or letter size paper;
except charts may be displayed in landscape mode.
5.2. Solicitation forms requiring to be completed shall be typewritten with fonts no smaller than 9 points.
5.3. No hyperlinks or electronic media will be allowed or considered.
5.4. Any conceptual drawings, renderings, product data sheets, one-line diagrams, etc. included in your proposal, shall be submitted as portable data files (pdf).
5.5. Strongly recommend formatting your technical proposal factor by factor, as listed in the solicitation. Stick to the page limits.
5.6. Proposals which merely restate the requirement or state that the requirement will be met, without providing supporting rationale, are not sufficient and will not be considered an appropriate response.
It is the offeror’s responsibility to insure the completeness of the proposal.
5.7. Offers that do not propose to perform all the work required by the solicitation will not be considered.
5.8. Proprietary Data: Each page of the offeror’s proposals must be reviewed and marked as to proprietary data content by the offeror in strict compliance with FAR 52.215-1. Also, see FAR 3.104-4. A single blanket statement at the front of the proposal is not acceptable. Failure to mark every page will subject the proposal to public release through Freedom of Information Act (FOIA) requests.
5.9. Proposals submitted in response to this solicitation shall be signed by the SDVOSB 51% owner and dated.

10. The proposed prices will be analyzed for fairness and reasonableness. Include copy of the Pricing
Schedule of the solicitation. Provide a detailed price breakdown for the purpose of evaluating price.
The breakdown shall separately at a minimum but not limited to (this is not an all-inclusive breakdown list, DO NOT limit your breakdown to this list) include the following cost elements:
Labor (Direct & Indirect), Materials, Material Markups, Material Overhead, Equipment, Subcontractor Cost, Other Direct Costs, Overhead Expenses, Indirect Cost, General & Administrative (G&A) expenses, and Profit. While developing your price proposal please make sure to include all applicable taxes, and government stamps cost.
11. NOTE: Price breakdown shall be indicative of showing at least 15 percent of the cost of the contract performance incurred for personnel will be spent on the concern's employees or the employees of other eligible service-disabled veteran-owned small business concerns in accordance with VAAR 852.219-10.
12. PRICE EVALUATION: The Government will initially review pricing independently from the factor evaluation. Price is not rated but will be analyzed for fairness and reasonableness. The Government may use various price analysis techniques and procedures to make a price reasonableness determination.

III. EVALUATION FACTORS FOR AWARD

The evaluation factors for PHASE ONE consist of Factor 1 - Technical Approach, Factor 2 – Technical Qualifications, Factor 3 – Relevant and Recent Past Performance, Factor 4 – Project Planning/Safety.
Each factor shall have equal importance.
PHASE ONE – EVALUATION CRITERIA,
FACTOR 1: TECHNICAL APPROACH
(30 pages max., excluding signed letters of commitments, and copy of licenses)
1. In a narrative format, diagrams and/or drawing, describe in detail how the offeror intends to meet or exceed all performance areas stated in the solicitation/statement of work/attachments that are related to Project 573A4-23-800. Provide technical methods and procedures that reflect a design and construction approach unique to the site and facility. NOTE: Vague, general narratives to include the restatement of the Statement of Work will be rated less favorably.
2. Schedule and minimizing downtime is crucial. Describe a plan to complete the project in a timely manner without causing disruptions to facility during normal business hours. Also, describe actions to be taken by the offeror if completion of work is anticipated to fall behind schedule. It is critical to the VA mission that services remain intact with minimal disruption.
3. Describe any technical difficulties that may be encountered in a project of this type. Provide a narrative of preventative measures or solutions to overcome these issues.

FACTOR 2 - TECHNICAL QUALIFICATIONS:

b) List of subcontractors.
c) Relevance to project stated in RFP.
d) Initial/Final award amount.
e) State any change orders if occurred during the project performance.
f) Pictures of completed work.
g) Project Contracting Officer Representative’s or Client’s name, phone number, and email address.
h) Past Performance Questionnaire/Evaluation (optional but highly recommended).
Note: Teaming Arrangements - Identify any projects that the two contractors have previously contractually teamed together to accomplish.
(i) Constructor/Key Subs and/or Designer: Include examples, within the last 3 years (5 years for design firms), of design/build projects, in which the designer and/or constructor or key trade subcontractors have been involved, if any.
(ii) Teaming Arrangements: If Offeror represents the combining of two or more companies for the purpose of this RFP, then each company must list project examples, related to its role for this project.
2. The projects shall be recently completed (completion date to be within 5 years of the solicitation release date), relevant, and meet or exceed the proposed magnitude and scope of the proposed project 573A4-23-603.
Note: The Evaluation Team may also use information obtained from Past Performance Information Retrieval System PPIRS http://www.ppirs.gov/ as a basis for evaluating this factor.
3. Preferable to discuss similar type projects in excess of $100,000.00.

FACTOR 4 - PROJECT PLANNING/SAFETY/IC/QC
1. Describe your plan for phasing this project
2. Describe approach to construction safety and infection control (IC).
3. Submit Quality Control (QC) monitoring procedures that will ensure a high level of construction quality that is applicable to the Specific and General requirements of the Statement of Work.
4. Develop and include procedures involving key subcontractors in the quality of the construction processes.
5. Provide the precautions the contractor will take to avoid damages to facility buildings (include pre-existing conditions) and surrounding properties and the remediation that will take place if damage is done.

PHASE TWO TECHNICAL PROPOSAL
FACTOR 1 – DESIGN METHODS
In a narrative format, diagrams, and/or drawings, describe the process required to ensure that the renovation of the building shall be in compliance with VA design guide and master specifications.
Describe the preliminary tasks required to be completed prior to the start of design.
Narrative: 4-page maximum (diagrams and concept drawings are not counted into page limitation)
FACTOR 2 – CONSTRUCTION METHODS
a) Describe renovation process, including the asbestos and lead testing and abatement that may be needed to ensure proper performance of all tasks.
b) Describe the A/E firm’s responsibility during construction phase and the management’s approach to effectively coordinate with the A/E and solve any issues in the design.
o Narrative: 4-page maximum
FACTOR 3 – PROJECT COORDINATION AND SAFETY
Provide a narrative that effectively describes in detail a technical approach of how the offeror will (a) coordinate all work, (b) remain on schedule, (c) avoid/minimize disturbance to the clinic/facility, and (d) maintain Infection Control, Interim Life Safety Measures, and overall safety.
o Narrative: 4-page maximum
FACTOR 4 –PROJECT SCHEDULE
Submit a preliminary (CPM) Critical Path Method schedule for the design and construction satisfying the requirements of this project. A practicable schedule showing the order in which the Contractor proposes to perform the work, and the dates on which the Contractor contemplates starting and completing the several salient features of the work (including design, acquiring materials and equipment, mobilization, demolition, installation, commissioning, etc.). The schedule shall be in the form of a progress chart (Gantt chart) of suitable scale to indicate appropriately the percentage of work scheduled for completion by any given point during the period. Most importantly, the schedule should show length of downtime required. The schedule shall show key milestone dates and switchover and start-up of equipment. The schedule is not included in the page limit.
To be considered technically acceptable, the proposed “draft” project schedule shall not exceed the project duration stated on the solicitation and include divisions of work required to successfully complete the project.
o Narrative: 5-page maximum (shall be Gantt chart).
FACTOR 5 – MANAGEMENT APPROACH, KEY PERSONNEL, QUALITY CONTROL, AND
WARRANTY SUPPORT
a) Describe any changes to management approach, key personnel, and quality assurance/control procedures that have occurred or will occur since Phase 1 proposals for this requirement were submitted.
b) Describe the organization, process, and procedures to be established, implemented, and carried out for warranty support.
Narrative - 2 page maximum
Phase 2 Proposal Submission: The Phase 2 proposal should be submitted to the same address as the Phase I proposal and can be submitted though use of the same format and means.
1. Phase 2 Evaluation and Selection Criteria:
Phase 2 proposals will be evaluated as follows:
a) The technical factors consist of the Factors 1 thru 5, described above and will be rated using the same combination system used for the Phase 1 proposal evaluation.
b) For evaluation purposes, each technical factor is equal in importance.
Phase 2 Evaluation and Award Determination
a) This is a competitive Phase 2, Request for Proposal (RFP) that represents the best value to the Government conducted under FAR Part 15 101-1, Tradeoff Process.
b) In accordance with FAR 15.101-1, all evaluation factors, other than cost or price, when combined, are approximately equal in importance to cost or price.
c) Thus, award will be made to the firm whose Phase 1 proposal was initially ranked in the top 5 and who now offers a Phase 2 proposal, which based on both its technical and pricing proposals, offers the overall best value to the Government.
d) Each pricing element indicated in TAB B will be evaluated to determine its reasonableness. The Government may use various price analysis techniques and procedures to make a price reasonableness determination. Offerors should assure that their ability to meet or exceed the minimum needs of the Government is adequately described in the offer. The Government reserves the right to evaluate proposals and award a contract without discussions (except clarifications as described in FAR 15.306(a)). Therefore, the offeror’s initial proposal should contain the offeror’s best terms from a price and technical standpoint. The Government considers it to be in its best interest to allow consideration of award to other than the lowest priced offeror or other than the highest technically rated offeror.
e) Evaluation Overview: The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The proposals must conform to the RFP's requirements and judged to represent the Best Value to the Government. The Best Value is the most advantageous offer, price and other factors considered, and consistent with the Government's stated importance of evaluation criteria. This may result in award being made to a high-rated, higher-priced offeror where the Contracting Officer determines that the technical capability of the higher-priced offeror outweighs the price.
f) SDVOSB Verification: The Government will review VetBiz
(https://www.vip.vetbiz.gov/) to confirm Service-Disabled Veteran-owned Small Business (SDVOSB) status verification for the submitted proposals. Evaluations will be performed on proposals submitted by verified SDVOSBs only. Subsequent confirmation of SDVOSB verification status will be performed for the apparent award.
Safety or Environmental Violations and Experience Modification Rate (EMR)
A. “All Offerors shall submit the following information pertaining to their past Safety and
Environmental record. The information shall contain, at a minimum, a certification that the offeror has no more than three (3) serious, or one (1) repeat or one (1) willful OSHA or any EPA violations(s) in the past three years.
B. All Offerors shall submit information regarding their current Experience Modification Rate (EMR) equal to or less than 1.0. This information shall be obtained from the offeror’s insurance company and be furnished on the insurance carrier’s letterhead.
C. Self-insured contractors or other contractors that cannot provide their EMR rating on insurance letterhead must obtain a rating from the National Council on Compensation Insurance, Inc.
(NCCI) by completing/submitting form ERM-6 and providing the rating on letterhead from NCCI. Note: Self-insured contractors or other contractors that cannot provide EMR rating on insurance letterhead from the states or territories of CA, DE, MI, NJ, ND, OH, PA, WA, WY, and PR shall obtain their EMR rating from their state-run worker’s compensation insurance rating bureau.
D. A Determination of Responsibility will be accomplished for the apparent awardee prior to processing the award. The above information, along with other information obtained from Government systems, such as the OSHA and EPA online inspection history databases will be used to make the Determination of Responsibility. If the apparent awardee fails to provide required documentation, then the matter will be referred to the Small Business Administration (SBA) office for a certificate of competency or a determination of responsibility from SBA.
E. This requirement is applicable to all subcontracting tiers, and prospective prime contractors are responsible for determining the responsibility of their prospective subcontractors. “ (Offerors to refer to EMR)

2.2 52.222-23 NOTICE OF REQUIREMENT FOR AFFIRMATIVE ACTION TO ENSURE
EQUAL EMPLOYMENT OPPORTUNITY FOR CONSTRUCTION (FEB 1999)
(a) The offeror's attention is called to the Equal Opportunity clause and the Affirmative Action Compliance Requirements for Construction clause of this solicitation.
(b) The goals for minority and female participation, expressed in percentage terms for the Contractor's aggregate workforce in each trade on all construction work in the covered area, are as follows:

These goals are applicable to all the Contractor's construction work performed in the covered area. If the Contractor performs construction work in a geographical area located outside of the covered area, the Contractor shall apply the goals established for the geographical area where the work is actually performed. Goals are published periodically in the Federal Register in notice form, and these notices may be obtained from any Office of Federal Contract Compliance Programs office.
(c) The Contractor's compliance with Executive Order 11246, as amended, and the regulations in 41 CFR 60-4 shall be based on (1) its implementation of the Equal Opportunity clause, (2) specific affirmative action obligations required by the clause entitled "Affirmative Action Compliance Requirements for Construction," and (3) its efforts to meet the goals. The hours of minority and female employment and training must be substantially uniform throughout the length of the contract, and in each trade. The Contractor shall make a good faith effort to employ minorities and women evenly on each of its projects. The transfer of minority or female employees or trainees from Contractor to Contractor, or from project to project, for the sole purpose of meeting the Contractor's goals shall be a violation of the contract, Executive Order 11246, as amended, and the regulations in 41 CFR 60-4. Compliance with the goals will be measured against the total work hours performed.
(d) The Contractor shall provide written notification to the Deputy Assistant Secretary for Federal Contract Compliance, U.S. Department of Labor, within 10 working days following award of any construction subcontract in excess of $10,000 at any tier for construction work under the contract resulting from this solicitation. The notification shall list the—
(1) Name, address, and telephone number of the subcontractor.
(2) Employer's identification number of the subcontractor.
(3) Estimated dollar amount of the subcontract.
(4) Estimated starting and completion dates of the subcontract; and
(5) Geographical area in which the subcontract is to be performed.
