- Be responsible for any injury to employees or damage to personal or public property.
3. Initial Containment of Infectious (Regulated) Waste and Chemotherapy Waste:
All infectious waste, except sharps, shall be bagged by the government in VA provided red disposable plastic bags which are impervious to moisture. Infectious waste of the “sharps” category, which originates from the diagnosis, care and treatment of patients that have been or may have been exposed to a contagious or infectious disease, or immunization or in the production or testing of biological, shall be disposed of by the medical center in hard plastic containers designed for that purpose.
The contained infectious waste shall be stored by the government in transport containers, as described below, at the designated storage site located inside Building no. 1 at the Omaha VA, inside Building no. 3 at the Lincoln VA, inside Building no. 1 at the Grand Island VA, Center Mall 5th floor (Whole Health), Center Mall 2nd floor (Polytrauma), Community Resource and Referral Center (CRRC), Norfolk CBOC, Shenandoah CBOC, Holdrege CBOC, and North Platte CBOC.
The use of a distinctive, clearly marked container with a biohazard symbol is essential.
Packaging must maintain its integrity during storage and transport to ensure containment of infectious waste. Waste may not be compacted or mixed with other waste materials prior to destruction and disposal.
Many items placed in hazardous waste disposal receptacles may contain PHI due to the placement and non-removal of identifying information. This includes patient labels on IV bags.
4. Transport Containers:
The Contractor shall provide clean, disinfected, rigid plastic containers to the Government at each pickup of stored regulated waste. The Contractor will provide an adequate number of barrels/containers, with an approximate capacity of 44 gallons, each week to the respective central storage areas of the VA Medical Centers in Omaha, Lincoln, Grand Island, Center Mall 5th floor (Whole Health), Center Mall 2nd floor (Polytrauma), Community Resource and Referral Center (CRRC), Norfolk CBOC, Shenandoah CBOC, Holdrege CBOC, and North Platte CBOC.
The containers will be lined with contractor provided 2 mil. Red plastic bags and have snug fitting lids. The containers are to be stackable. The Contractor shall provide the quantity of containers as needed and will exchange a clean disinfected container for each filled container, picked up daily, six days per week in Omaha, picked up twice per week in Grand Island. The Contractor will also provide the quantity of containers as needed and will exchange a clean disinfected container for each filled container, picked up monthly at the Center Mall 5th floor (Whole Health), Center Mall 2nd floor (Polytrauma), Community Resource and Referral Center (CRRC), Norfolk CBOC, Shenandoah CBOC, Holdrege CBOC, and North Platte CBOC.
Whenever full containers are picked up at any site, enough disinfected empty containers will be left in exchange. Currently, the volume of regulated (infectious) waste generation at the Omaha Division is 125,000 pounds per year; 12,000 pounds per year in the Lincoln Division and 14,500 pounds per year at the Grand Island Division. All containers shall be prominently labeled with the word “BIOHAZARD” and the international biohazard symbol.
The Covered Entity’s intention is not to disclose PHI and the vendor only transports the information but does not access it.
5. Pick Up and Transport:

iii. All labor to remove filled containers from the storage, load the truck, and replace with empty, clean disinfected containers in designated areas for each site is the sole responsibility of the Contractor.
iv. At the conclusion of each pick-up visit, the Contractor shall provide the Contracting Officer’s Representative (COR) at each site, dated written documentation of how many filled storage containers were loaded for transport signed by EMS Staff only. A document of destruction shall be provided for each pick up from each site and shall be followed by a certificate of destruction stating total poundage destroyed.
b. The vehicle shall be identified as follows:
i. Exterior identification conspicuously displaying a rectangular sign or decal at least 12inches in size with red letters on white background which reads “INFECTIOUS WASTE” or a decal displaying the international biohazard symbol and the word “BIOHAZARD”.
ii. Name and telephone number of the company owning the vehicle on each side and back of the vehicle in accordance with all Local, State and Federal regulations regarding infectious waste.

vii. The procedures to follow in the event that load of infectious waste becomes hot, burning and/or reactive if the lower flammable (L.F.L.) limit is exceeded.
viii. The procedures to follow in the event unauthorized waste material is deposited in the collection containers for infectious waste.
ix. The procedures to follow to complete the manifest and to maintain other records required under any ordinance or under federal regulations.
x. The procedures to follow to properly disinfect the infectious waste collection vehicle.
The Covered Entity is notifying the vendor of the possibility of PHI being incidentally disclosed as a by-product of their services and obtain acknowledgement from the vendor that the hazardous waste receptacles (bags) will not be opened.
6. Destruction and Disposal:
The Contractor is fully responsible for destruction and disposal of the infectious waste and Chemotherapy waste generated by the Department of Veteran Affairs Nebraska-Western Iowa Health Care System Omaha Division, Lincoln Division and the Grand Island Division, Center Mall 5th floor (Whole Health), Center Mall 2nd floor (Polytrauma), Community Resource and Referral Center (CRRC), Norfolk CBOC, Shenandoah CBOC, Holdrege CBOC, and North Platte CBOC. The waste is to be destroyed in accordance with all Local, State and Federal regulations regarding infectious waste. The Contractor is individually and solely responsible for complying with all governmental regulations regarding regulated (infectious) and solid waste.
Disposal of destroyed infectious waste shall be only at an approved disposal site as required by the local governmental authority having jurisdiction.
Disposal of Non-paper Items: Non-paper items (i.e. I.V. bags, wristbands, prescription bottles, etc.) containing III must be de-identified prior to disposal. The identifiable information taken off of these items must be placed in the approved shred bins. If this is not possible the material with III needs to be put in with the red biohazard bins for medical waste. Per VHA Directive
1605.06 Safeguarding and Disposing of Patient Wristbands In VA Medical Facilities, staff are required to document in the patient discharge note that the wristband was removed and how it was destroyed or that the patient refused to have wristband removed at discharge and that they were educated on the risk of improper disposal of patient wristband containing PII and PHI.
The acceptable process for destruction of this material such as incineration is to ensure that any PHI in hazardous waste bags is fully destroyed so that it is not readable or re-constructable.
7. Weighing System:
The Contractor will provide a certifiable weighing system to enable proper billing for waste poundage disposed; i.e., if the disposal container weighs 14 pounds and the Contractor is to dispose/haul 10 containers, each container should be weighed and the container total weight
(140 pounds) is subtracted from the gross weight to determine total waste poundage to be paid.
8. Permits:
The Contractor must obtain all necessary permits and/or licenses regarding the pickup and disposal of contaminated medical waste as regulated by State and Federal EPA Guidelines for biohazardous medical waste. The Contractor/transporter agrees to hold the Government harmless for any negligence of the Contractor/transporter or his/her employees who may wholly or individually be liable for improper disposal during the performance of this contract.
9. Documentation:
