About this file

This is a Past Performance Questionnaire (PPQ) template designed for evaluating contractor performance on federal construction projects under the Department of Veterans Affairs Veterans Health Administration (VHA) solicitation 36C25526R0160 for Project 589A7-23-305, "Prepare Site for Choice Dining Equipment" at the Robert Dole VA Medical Center in Wichita, Kansas. The form is completed by the contractor being evaluated in Section 1, which requires documentation of the offeror's name, address, contact information, work performed percentage, prime contractor information (if applicable), contract details including contract number, type, location, award and acceptance dates, implementation and final contract prices, and a description of project scope and relevance to the current submission.

Section 2 is completed by the client or project evaluator and requests their role description and performance assessments across seven key evaluation categories: quality of design and construction standards; schedule compliance and timeliness of deliverables; communication and cooperation; management effectiveness and personnel continuity; cost and financial management; subcontract management; and an overall summary rating with recommendation for future engagement. Evaluators select from five adjectival rating levels—Outstanding, Above Average, Satisfactory, Unsatisfactory, or Not Applicable—with detailed definitions provided to ensure consistent assessment of the contractor's ability to perform work as described in the solicitation. The completed PPQ is to be returned to the offeror for incorporation into the SF330 submission rather than submitted directly to VHA NCO 15.
