About this file
This is a Task Order Template for Insurance Entry and Verification (IV) Buffer Support Services issued under a parent Indefinite-Delivery/Indefinite-Quantity (IDIQ) contract. The template establishes the framework for ordering contractor services to process insurance actions for the Department of Veterans Affairs (VA) across multiple Consolidated Patient Account Centers (CPACs) and VA Medical Centers (VAMCs).
The contractor shall provide two authorized action types: reverification actions and new-policy actions, including Medicare policy actions. Services must be completed within specified delivery periods, with a baseline standard of at least 93 percent completion within the applicable delivery period and individual data-entry quality reviews not falling below 95 percent. The Task Order uses firm-fixed-price compensation with unit prices derived from the parent IDIQ contract. Payment is limited exclusively to accepted completed actions; contractor-caused rejected actions and non-chargeable exceptions are not billable. The contractor must provide all necessary staffing, supervision, quality control, internal processes, equipment, facilities, and connectivity, except for expressly identified Government-furnished resources such as system access, training, and credentialing support. Reporting requirements include monthly itemized statements and staffing reports due by the fifth day of each month, and weekly pending workload reports due by Monday of each week. The template requires identification of covered CPACs/VAMCs, period of performance (within a 36-month ordering period), workload estimates, delivery periods, priorities, Government points of contact, and any Task Order-specific procedures or facilities. A pre-issuance checklist ensures compliance with parent contract scope, pricing, funding, and performance standards before issuance.

Government receipt or review of a recurring report does not by itself establish acceptance of underlying actions or authorize payment.
