About this file

This is a Past Performance Questionnaire (PPQ) template used by the federal government to collect and evaluate contractor performance on previous government contracts.

The form is divided into two main sections: contractor-completed blocks (1-4) and client-completed blocks (5-8). The contractor provides basic company information including firm name, CAGE code, address, contact details, and work role (prime contractor, subcontractor, joint venture, or other). The contractor also furnishes contract specifics including contract number, type (firm fixed price, cost reimbursement, or other), title, location, award date, completion date, original contract price, and final contract price with any modifications. The contractor describes project complexity (high, medium, or routine) and provides relevant project details. The client then completes the questionnaire by providing their name, title, contact information, and role description, along with the completion date and signature.

The evaluation section contains seven performance rating categories: Quality, Schedule/Timeliness of Performance, Customer Satisfaction, Management/Personnel/Labor, Cost/Financial Management, Safety/Security, and General. Within each category, clients rate specific performance elements using a six-point scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or Not Applicable). Page 2 provides detailed definitions for each rating level with specific guidance on when each rating is appropriate. The form concludes with yes/no questions regarding contractor accounting adequacy, contract termination history, and financial problems, and asks whether the client would rehire the contractor. Clients are requested to provide narrative comments on contractor strengths, weaknesses, and deficiencies to support their performance risk evaluation.
