B.2 STATEMENT OF WORK

STATEMENT OF WORK

Garland VA Boiler & Chiller Plant Services

1.0 SCOPE OF WORK

This contract is for Testing & Inspecting, Preventative Maintenance, Corrective Repair, Emergency Repair, and Service Calls for the Boiler/Chiller Plant at the Garland VA Medical Center.

Contract is needed to maintain the boilers, chillers, and CHP Plant operating in a safe and efficient manner and ensure compliance with emission requirements as set forth by the Texas Commission on Environmental Quality (TCEQ) permit.

Each piece of equipment and tasks, and schedule are identified in Appendix A. Detailed Tasking Sheets for each piece of equipment is detailed in the Appendix.

1.1 Place of Performance

Garland VA Medical Center 2300 Marie Curie Boulevard Garland, TX 75042

1.2 Period of Performance

Base Year: November 15, 2024 – November 14, 2025 Option Year 1: November 15, 2025 – November 14, 2026 Option Year 2: November 15, 2026 – November 14, 2027 Option Year 3: November 15, 2027 – November 14, 2028 Option Year 4: November 15, 2028 – November 15, 2029

1.3 SERVICE HOURS REQUIREMENTS:

Contractor must be able to provide:

- Testing & Inspecting & Preventative Maintenance

Testing and inspections, preventative maintenance, and corrective repairs will be conducted during normal business hours of 8:00 a.m.- 5:00 p.m. Monday – Friday, excluding Federal Holidays.

1.4 EMERGENCY CALLS RESPONSE

Emergency service calls request and requirements are to be acknowledged by:

(1) one hour by phone.

(2) two-hour onsite response, 24 hours/day, 7 days a week.

1.5 ROUTINE SCHECULE SERVICES AND CORRECTIVE REPAIRS:

Contractor will restore equipment to full service within forty-eight (48) hours of arriving on job site. The Contracting Officer Representative (COR) will determine extenuating event(s) that may require more time without penalty to the contractor. Fully serviceable means the defective part(s) have been replaced with parts that meet or exceed the manufacturers original performance specifications

A certified technician must be on site within 12 hours of acknowledgment of service call.

2.0 CONTRACTOR CERTIFICATIONS

Qualified technicians must be certified and have 4 years minimum experience working on the equipment. Contractors must have the following certifications per equipment.

• Allen Bradly or current system

• Certified Chiller Technicians

• Boilers- Contractor has documented knowledge and experience in the VA’s standards for construction and testing of safety devices. Technician shall have demonstrated experience in conducting equipment assessments and performing testing in compliance with the VA Safety Device Testing Manual on at least 5 sites with 3 years - VHA 1810.

• Fireye boiler controls or current burner management system

• High/Med/Low voltage electrical equipment.

3.0 DESIGNATED FULL TIME PROJECT COORDINATOR

Contractor will provide a full-time project coordinator to provide ONSITE MANAGEMENT of this contract.

• The Project Coordinator will oversee, direct, coordinate activities of the service providers and sub-contractors, and will be responsible for maintenance coordination of directing, planning, remote troubleshooting provided by service providers and subcontractors.

• The Project Coordinator will organize & schedule for the site related issues for the maintenance, corrective repairs, and emergencies. Includes scheduling service providers for all services covered in this contract per the equipment maintenance schedule and/or provided based on equipment, location, application, run time.

• Project Coordinator is responsible to assure that all contractors follow check in & check out processes per VA standards.

• The project coordinator will manage and coordinate all documentation per VA standards

& procedures in testing section This preventative maintenance program is based on the O&M manufacturer’s recommendation and the project coordinator will check with the O&M manufacturer annually for any changes/recommendations to the preventative maintenance program and adjust accordingly.

3.1 CONTRACTOR FULL-TIME PROJECT COORDINATOR REQUIREMENTS:

- Project Coordinator experience.

- Must have 8 - 10 years in Electrical, HVAC, or Boiler/Chiller Plant operations.

4.0 CONTRACTOR PROCUREMENT CAPABILITES

Contractor will be required to establish all subcontractors are in VA vendor profile. Contractor will ensure work will be performed within 90 days of signed agreement and must be able execute active funding to subcontractors within 5 days for special projects/emergencies. Sub- Contractors plan must be able to mirror agree/operate under VA’s terms and conditions of this contract.

5.0 PREVENTATIVE MAINTENANCE

This program covers preventative maintenance and includes on site labor, travel labor, travel, living expenses, tools, maintenance parts procurement (locating, ordering, expediting, and transporting) and equipment such as cranes, scaffolding, and rigging.

All maintenance materials, parts, seals etc. necessary to perform preventative maintenance tasks outlined in Exhibit A (maintenance) are provided by the contractor as part of this contract.

Repair labor, repair parts, emergency labor and emergency parts are defined and directed in Paragraph 6.0 Program Definitions.

6.0 PROGRAM DEFINITIONS.

6.1 TEST & INSPECT: Inspections include on site labor, travel labor and travel and living expenses to visually INSPECT and TEST equipment to determine its operating condition and efficiency. Typical activities include:

6.2 TESTING for excessive vibration, motor winding resistance, refrigerant charge, refrigerant oil (acid) water condition, flue gas analysis, combustion and draft, equipment control systems, and boiler burner management and control safety devices etc.

6.3 INSPECTING for worn, failed or doubtful parts; mountings, drive couplings, oil level;

rotation; flame composition and shape, pilot, and igniter; steam, water, oil and/refrigerant leaks, etc.

6.4 RECORDING – as per tasking sheets.

6.5 INCLUDED - Annual Boiler Testing is in accordance with the current edition of VHA Boiler and Associated Plant Safety Device Testing Manual. The Project Coordinator will review and consolidate all service documentation as directed by the COR as per tasking sheets.

7.0 PREVENTATIVE MAINTENANCE:

Preventative Maintenance includes on site labor, tools, travel labor and travel and living expenses required to clean, align, calibrate, tighten, adjust, lubricate. These activities are intended to extend equipment life and assure proper operating condition and efficiency. All maintenance materials and parts are provided by the contractor. All removed parts are to be turned over to the VA for inspection.

Typical activities include:

• CLEANING coil surfaces; fan impellers and blades; electrical contacts; burner orifices;

passages and nozzles; pilot and ignitor; level float; chiller; condenser and boiler tubes;

etc.

• ALIGNING belt drives; drive couplings; coil fins, etc.

• CALIBRATING safety controls; temperature and pressure controls and sensors etc.

• TIGHTENING electrical connections/terminations; mounting bolts; pipe clamps;

refrigerant piping fittings; damper sections; etc.

• ADJUSTING belt tension; refrigerant charge; super heat; fan RPM; water chemical feed and feed rate; burner fuel/air ratios; gas pressure; set point of controls and limits;

compressor cylinder unloaders; damper close off, sump floats, etc.

• LUBRICATING motors; fan and damper bearings; valve stems; damper linkages; fan vane linkages, etc.

7.1 CORRECTIVE REPAIR, EMERGENCY REPAIR HOURS & PARTS

All corrective maintenance/repair and emergency/repair labor outside the scope of preventative maintenance defined in Paragraph 5.0 will be drawn from a Not-to-Exceed amount that may increase or decrease depending on number and price of repair(s). (For normal business hours and emergency).

All corrective repair/emergency repair includes on site labor, travel labor, travel and living expenses, tools, maintenance parts procurement (locating, ordering, expediting, and transporting) and equipment such as cranes, scaffolding, and rigging.

See Paragraph 7.4 PARTS & PARTS SPECIFICATIONS regarding parts for corrective repair/emergency service.

7.2 REPAIRS & CORRECTIVE MAINTENACE

Repairs & Corrective Maintenance is to restore equipment, a machine, or a system to an operational condition and includes troubleshooting, disassembly, adjustment, repair, replacing failed/defective parts, and alignment so it can perform it’s intended function. Repairs & Corrective Maintenance will be drawn from a Not-to-Exceed amount that may increase or decrease depending on number and price of repair(s). (For normal business hours and emergency).

Any parts required for repair/corrective maintenance will be purchased by the VA. See parts requirements section 7.3.

• Contractor has the option to conduct repairs at an offsite location or VANTXHCS at no additional cost to the VA.

• If a part or component has become obsolete preventing the contractor from supporting or repairing the equipment, the contractor will notify VANTXHCS before any further work is completed and will be advised how to proceed by the VA.

7.3 PACKAGE PARTS AND CONSUMABLES

Contractor will provide certified manufacturer replacement parts to cover defects in workmanship. Equipment replacement must be proprietary per manufacture.

7.4 PARTS & PARTS SPECIFICATIONS

• Any parts/materials outside the scope of testing, inspecting, and preventative maintenance will be approved and supplied by the VA. Corrective Repair and/or Emergency Repair parts will be purchased by the VA or will be purchased by the contractor upon approval of the VA and drawn from the $100,000.00.

• Contractor reserves the right to replace any component or part with either a new or manufacturer rebuilt part, with the exception of the Burner Management System for the boilers or replace complete assemblies instead of individual components, and must be approved and purchased by the VA.

• No refurbished parts that are not provided by the manufacturer.

• Equipment replacement must be proprietary per manufacture.

8.0 LOCK OUT TAG OUT PROCEDURES

Subcontractors are responsible to make sure equipment is Locked Out Tagged Out (LOTO) in accordance with VA approved LOTO procedures.

Annual Electric Chiller Operational Tests & Inspections - Preventative Maintenance

Electric Driven, Centrifugal 900-ton units. Model: York YK (2ea R-22 1ea R-134a)

YORK YK. To be completed before May 1 of each year. Contractor shall provide all consumables, personnel, transportation, supplies, cranes, rigging, scaffolding and labor as required to successfully complete the Annual Preventative Maintenance inspections. Required tasks/check points are outlined below. One (1) report per each chiller is required and shall contain pertinent results and/or comments along with any recommendations for corrective maintenance actions. Contractor shall log any circumstances that may prevent completion of an inspection.

NOTE: These PM inspections are to be scheduled within the same 2 – 4-week time frame each year; unless otherwise directed by the COR.

Boilers: Annual Service, Cleaning, and Testing

Contract covers a total of 2 Cleaver-Brooks fire tube boilers.

To be performed between May 1 and October 31 and must be completed before November 1 of each year.

Required tasks/check points are outlined below. One (1) report per each boiler is required and shall contain pertinent results and/or comments along with any recommendations for corrective maintenance actions. Contractor shall log any circumstances that may prevent or delay completion of a task.

C.2 SUPPLEMENTAL INSURANCE REQUIREMENTS

In accordance with FAR 28.307-2 and FAR 52.228-5, the following minimum coverage shall apply to this contract:

(a) Workers' compensation and employers liability: Contractors are required to comply with applicable Federal and State workers' compensation and occupational disease statutes. If occupational diseases are not compensable under those statutes, they shall be covered under the employer's liability section of the insurance policy, except when contract operations are so commingled with a Contractor's commercial operations that it would not be practical to require this coverage. Employer's liability coverage of at least $100,000 is required, except in States with exclusive or monopolistic funds that do not permit workers' compensation to be written by private carriers.

(b) General Liability: $500,000.00 per occurrences.

(c) Automobile liability: $200,000.00 per person; $500,000.00 per occurrence and $20,000.00 property damage.

(d) The successful bidder must present to the Contracting Officer, prior to award, evidence of general liability insurance without any exclusionary clauses for asbestos that would void the general liability coverage.

(d) Comptroller General Examination of Record. (1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.

(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract.
