INVOICES:
Payments shall be made in arrears upon receipt of a properly prepared invoice.
In order to comply with the Improper Payment Elimination and Recovery Act of 2010 (IPERA), the VA has mandated electronic invoice submission to the Veterans Affairs Financial Services Center (VAFSC).
VAFSC has partnered with Tungsten Network e-Invoicing network, for submissions of all electronic invoices to VA. Tungsten Network electronic invoicing is free to all VA vendors. In order to submit electronic invoices, all VA vendors must register with Tungsten Network by submitting an email to

Contractor shall submit an electronic invoice by the tenth (10th) of the following month services were performed to the Veterans Affairs Financial Services Center (VAFSC) e-Invoice through the website at https://portal.ob10.com/Login.aspx. All invoices shall reference the vendor name and address, customer name, contract number, appropriate obligation/funding order number, description of services provided, quantity, unit price, and total invoice amount (any additional info). Invoices shall also include any payment discount terms.

SPECIAL NOTES
In order to maintain integrity of this solicitation and subsequent award date, all offerors are advised that any question and answer requests must be submitted in a written form via e-mail to the Contracting Officer susan.pasholk@va.gov no later than THURSDAY MAY 28, 2020 AT
NOON LOCAL. Contracting will issue a consolidated response via solicitation amendment to www.fbo.gov within 2 business days to all parties.
The Contracting Officer is the only person authorized to approve changes or modify any of the requirements under this contract. The Contractor shall communicate with the Contracting Officer on all matters pertaining to contract administration. Only the Contracting Officer is authorized to make commitment or issue changes that will affect price, quantity or quality of performance of this contract. In the event the Contractor effects any such change at the direction of any person other than the Contracting Officer, the change shall be considered to have been made without authority and no adjustment will be made in the contract price to cover any increase in cost incurred thereof.
SECURITY & PRIVACY CONTROL:
The Certification & Accreditation (C&A) requirements of VHA Handbook 6500.6 do not apply to this requirement, therefore, a Security Accreditation Package is not required. AUTHORITY
This Request for Quote (RFQ) solicitation is issued under the authority of FAR Part 13.5 for Certain Commercial Items in which simplified acquisition procedures in FAR Part 13 will be used. FAR Part 14 and 15 will not be used.
The Lowest Price Technically Acceptable source selection process will be used for this FAR
13.5 procurement. This means award will be made on the basis of the lowest evaluated price of quotes meeting or exceeding the acceptability standards for non-cost factors. See 52.212-2 Evaluation – Commercial Items (Section E) for a description of the factors in addition to price that the Government will use to evaluate offers. Prospective contractors must furnish the information listed herein with their quotations to ensure they receive full consideration.
SITE VISIT
It is strongly suggested and expected that the offeror inspect the campus to be serviced to fully understand the nature of the work and the conditions under which the work is to be performed.

ROOF TIEBACK TESTING AND INSPECTION
WILLIAM S. MIDDLETON VA HOSPITAL
STATEMENT OF WORK
The contractor shall provide all labor, materials, tools, and supervision to perform the required testing and inspection to include certification of roof tieback anchors at the William S. Middleton VA Hospital, 2500
Overlook Terrace, Madison, WI 53705-2286.
1. GENERAL SCOPE
A. The Madison VA Hospital consist of multiple buildings, structures and associated roofs. These roofs, installed during past half a century, have tieback anchors and fall protection provisions to a limited degree. These tiebacks and fall protection anchors are used as needed basis for window washing, repair and equipment inspection programs.
B. Current federal and industry regulations require specific load rating of tiebacks with a testing, inspection, certification, and maintenance program.
C. The existing roof tieback anchors are planned to be supplemented by new anchors. Installation of new anchors identified in drawings shall not be included in this contract. The existing and proposed new anchors shall become part of this contract for periodic Inspection and Certification
Program for succeeding years.
2. SPECIFIC TASKS – BASE YEAR
A. Contractor shall conduct Annual Inspection, load testing and certification of both existing and new anchors. Contractor shall provide recommendation for the repair of any failures.
B. The load testing criteria and rating shall satisfy the stricter of load testing requirements established for fall arrest or equipment protection:
• Anchorages.- 1910.27(b)(1)(i)- Before any rope descent system is used, the building owner must inform the employer, in writing that the building owner has identified, tested, certified, and maintained each anchorage so it is capable of supporting at least 5,000 pounds in any direction, for each employee attached. The information must be based on an annual inspection by a qualified person and certification of each anchorage by a qualified person, as necessary, and at least every 10 years.
• 1910.66(g) Inspection and Tests: The anchorage should be rigid and should not have a deflection greater than 0.04 inch when a force of 2250
Lbs is applied.
C. Refer to drawings in Appendix A indicating the location of existing anchors. The total number of existing anchors are counted as 143; the total number of new anchors are counted as 144; for a total number of 287 anchors.
D. The testing load shall not be less than 5,000 lbs per employee attached.
E. Contractor shall coordinate the inspection and testing with the Engineering Department at the VA
Madison Hospital regarding access, timing and local regulations regarding safety and working conditions.
F. The procedure/process for testing that is used by the Contractor shall be certified by a professional engineer.
G. The Contractor shall meet with the COR to review the results and recommendations within 15 days of completion of Annual Inspection and Test.
3. SPECIFIC TASKS – OPTION YEARS
A. The contractor shall provide annual certification of adequate load bearing capacity of all existing and new anchors installed (under a separate contract). The total number of anchors
(existing and new) is 287.
B. A visual inspection of all anchors shall be conducted by a qualified person.
C. The Contractor’s fee schedule and payment shall be based on a pro-rata adjustment of final quantity of new anchors.
D. Random testing of 5% of all anchors shall be conducted every year by qualified personnel.
E. Certification shall be per paragraph 6 Required Documentation.
F. Each Option Year, the Contractor shall meet with the COR to review the results and recommendations within 15 days of completion of Annual Inspection and Test.
4. SCHEDULE
A. The Contractor shall complete Load Testing of Existing Tieback Anchors and report within 30 days of award of this contract.
B. The Schedule for Load Testing of New Tieback Anchors is dependent upon the construction contract. All work will occur during the base year. The Contractor shall complete Load
Testing of New Tieback Anchors and report within 30 days of notification of new anchors being ready for testing.
C. Option Years: The selected contractor shall complete the Periodic Inspection, Testing and
Certification Program by December 31 of each option year, to be coordinated in advance with the COR.
5. FACILITY CHECK IN/OUT:
Coordinate with COR for access, parking and normal business hours of operation. All work is intended to occur during normal business hours.
6. REQUIRED DOCUMENTATION
Upon completion of work, contractor will provide The Certification of adequate load bearing capacity of each anchor to include:
• Qualification of person(s) performing the testing
• Testing of each anchorage of supporting at least 5,000 lbs, in any direction, for each employee attached.
• Recommendations for and implementation of corrections or replacement of each defective or damaged anchorage;
• Certification that each anchorage meets the requirements in §1910.27(b)(1)(i); by a licensed professional engineer
• The testing procedure/process shall to be certified by a professional engineer
• A date the contract work is completed.
7. IDENTIFICATION PARKING SMOKING AND VA REGULATIONS
A. No contractor or sub-contractor employee shall be permitted to perform work or access non-public areas unescorted without an ID card obtained from the VA Police. Contractor's and any
Subcontractor's employees will be required to provide 2 forms of ID and be accompanied by the
COR to obtain ID.
B. It is the responsibility of the Contractor to park in the appropriate designated parking areas.
Information on parking is available from the VA Police Section. The VAH will not invalidate or make reimbursement for parking violations of the Contractor under any conditions.
C. Effective January 1, 2019, smoking is prohibited both inside and outside (entire campus) at the
VAH.
D. Possession of weapons is prohibited. Enclosed containers, including tool kits, shall be subject to search. Violations of VA regulations may result in citation answerable in the United States
(Federal) District Court, not a local district, state, or municipal court.
8. PROTECTION OF GOVERNMENT PROPERTY
During work execution, the Contractor shall take special care to protect Government property including furniture, walls, baseboards, and other surfaces. Accidental splashes shall be removed immediately. Damage resulting from the Contractor operations shall be repaired by the Contractor, including painting, refinishing, or replacement, if necessary, at no additional cost to the Government. The Contractor shall be responsible to meet OSHA/Safety requirements in the performance of the work. This shall include, but not limited to, taking all the necessary precautions to protect the patients, visitors, and/or staff at each location. The
Contractor shall be held responsible for any injuries and /or damage, which may be caused as a result of the Contractors failure to adhere to these requirements.
Extreme caution shall be exercised to prevent damage to the building and its contents. Any damage must be reported to the COR immediately for appropriate action. Any damage caused by the contractor will be repaired and/or replaced to the satisfaction of the VA at the
Contractor’s expense.
9. COMPETENCY OF PERSONNEL SERVICING EQUIPMENT
A. Each respondent must have an established business, with an office and full-time staff. The staff includes a “fully qualified” FSE and “fully qualified” FSE who will serve as the back-up and be regular employees of the contractor.
B. “Fully Qualified” is defined for the purposes of this contract as having at least three (3) years of experience, within the last five (5) years performing this same service.

The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 30 calendar days prior to the contract expiration.

(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.

The Contractor shall use reasonable care to avoid damaging existing buildings, equipment, and vegetation on the Government installation. If the Contractor’s failure to use reasonable care causes damage to any of this property, the Contractor shall replace or repair the damage at no expense to the Government as the
Contracting Officer directs. If the Contractor fails or refuses to make such repair or replacement, the
Contractor shall be liable for the cost, which may be deducted from the contract price.
