About this file
This is a Certified Disadvantaged Business Outreach Plan attachment for Request for Proposal #6251, Obligation Number S-44300-00016445, issued by the Oregon Department of Administrative Services (DAS) Procurement Services for a System of Care Fiscal Assessment project. The procurement seeks to inventory and assess data related to Federal, state, and local funding streams, including expenditures and dedicated funds and revenues, to identify funding allocations benefiting youth ages 0-25 with complex behavioral health needs and to align funding models with the System of Care Advisory Committee's (SOCAC) strategic vision. The document functions as a mandatory disclosure form that proposers must complete and submit with their proposals, requiring identification of the proposer's status as an Oregon-certified firm and documentation of past contractual relationships with certified and non-certified small businesses.
The outreach plan requires proposers to describe steps they will take to solicit Certified Firms—including minority-owned businesses, woman-owned businesses, service-disabled veteran-owned businesses, and emerging small businesses certified under ORS 200.055 by the Oregon Certification Office for Business Inclusion and Diversity (COBID)—for subcontracting opportunities. Proposers must identify anticipated subcontracting opportunities with approximate dollar values and confirm willingness to report subcontractor identities and values to COBID if awarded the contract. The information submitted in response to this clause is not scored in the evaluation process; however, submission of the completed form is mandatory for all proposals.
