About this file

This is a Proposer Information and Certification attachment document for the System of Care Fiscal Assessment Request for Proposal issued by the State of Oregon. The document establishes the framework for proposal submissions and certifications required from bidders responding to this fiscal assessment initiative, which seeks to inventory and assess Federal, state, and local funding streams benefiting youth ages 0-25 with complex behavioral health needs served through Oregon's System of Care. The proposal process requires bidders to provide detailed organizational information, demonstrate relevant qualifications and experience, and execute required certifications affirming compliance with applicable regulations and contractual obligations. Specific submission requirements, response timelines, and evaluation criteria are outlined within this certification document to ensure consistent and transparent proposal review.

The attachment document establishes proposer eligibility requirements, mandatory certifications, and compliance declarations necessary for consideration in the competitive bidding process. Proposers must certify their organizational standing, confirm the accuracy of submitted information, and acknowledge adherence to state and federal requirements. The document may include provisions regarding insurance requirements, conflict of interest disclosures, non-discrimination certifications, and other standard contractual assurances required by the State of Oregon. By executing these certifications, proposers confirm their ability to meet the scope of work requirements for conducting the comprehensive fiscal assessment and identifying opportunities to align funding models with the System of Care Advisory Committee's strategic vision for Oregon's children and youth services.
