About this file

This is a Notice of Award issued by the Oregon Department of Human Services (ODHS) Shared Services Office of Contracts and Procurement on behalf of the Oregon Health Authority (OHA) for Request for Grant Proposals (RFGP) #S-44300-00015052 regarding the Jackson County Mobile Dental Van Expansion grant opportunity. The solicitation sought proposals from dental care organizations, Federally Qualified Health Centers (FQHCs), private dental clinics, and other healthcare organizations to augment or create a mobile dental van program addressing oral health disparities in Jackson County through comprehensive preventative oral health services. Proposers were required to submit applications by December 12, 2025, at 3:00 p.m. PST through the OregonBuys e-procurement system, with questions due by December 3, 2025, at 1:00 p.m. The grant evaluation panel reviewed proposals from December 19, 2025, through May 16, 2025, and La Clinica Del Valle Family Health Care Center, Inc d.b.a. La Clinica was selected as the apparent successful awardee, with notification issued on December 23, 2025. Grant agreement development was scheduled to occur from December 24, 2025, through December 31, 2025.

The OHA will provide a one-time payment of $70,000 to the selected recipient to procure equipment, recruit staff, and subsidize oral health service costs. Grant recipients must maintain $1,000,000 per occurrence and $2,000,000 in aggregate Commercial General Liability and Professional Liability Insurance and be registered with the Oregon Secretary of State Business Registry. The grant is supported by federal funds under Assistance Listings number 93.236 and requires compliance with federal and state requirements, including Medicaid services standards and comprehensive record-keeping for six years following final disbursement. Recipients must attend bi-monthly check-ins with OHA staff, provide annual expenditure and patient utilization reports, submit a detailed 24-month sustainability plan, and develop an evaluation framework to be submitted semi-annually. The OHA demonstrates a preference for entities with established mobile dental van programs elsewhere in Oregon and requires organizations to commit to non-discrimination guidelines and demonstrate financial investment beyond the grant funds sought.
