About this file

This document contains a past performance questionnaire template for a federal contractor to submit references from previous contracts. The questionnaire requests information on the contractor's role, a project description, contract period and amount. It includes a rating scale for the reference to evaluate the contractor's performance on factors such as quality control, schedule adherence, management and reporting. The related federal contract opportunity notice is a solicitation from the Department of the Navy for rolling admission to an indefinite delivery/indefinite quantity multiple award contract for ship repair and modernization services at the Mayport, Florida homeport. The solicitation seeks sources capable of supporting maintenance for various ship classes including cruisers, destroyers, landing platforms and docks. The estimated maximum value per contract is $50 million with a minimum guarantee of $10,000. Offerors must demonstrate ship repair experience and capacity to coordinate availabilities, and either possess a Master Ship Repair Agreement or equivalent facilities. The procurement will be competed among vendors capable of performing the work in the Mayport area in accordance with the Homeport Statute.
