About this file
This is an Applicant Information and Certification Sheet attachment for RFA S-10000-00017152, a joint cooperative procurement between the Oregon Department of Human Services (ODHS) and Oregon Health Authority (OHA) for telephonic interpretation services. The RFA seeks qualified vendors to provide scheduled and on-demand telephonic interpretation services ranging from client office visit support to high-volume call center operations for ODHS, OHA, and contracted partners' clients. The submission requires applicants to complete this form with legal entity name, contact information, state of incorporation, entity type, and Oregon Business Registry Number. Applicants must acknowledge receipt of all RFA addenda, submit a firm offer valid for 180 days following the closing date, and agree to perform the statement of work and meet performance standards as outlined in the final contract terms detailed in Attachment F "Sample Contract."
The certification sheet establishes mandatory compliance requirements for applicants, including attestations regarding tax law compliance, non-discrimination in employment and subcontracting practices, wage equity compliance under ORS 652.220, and adherence to Oregon Laws 2018 Chapter 88 regarding broadband Internet access services. Applicants must verify they are not listed on the Treasury Department's Specially Designated Nationals and Blocked Persons list or federal exclusion lists maintained in the System for Award Management. Applicants must certify the absence of conflicts of interest, confirm application accuracy and independent preparation without collusion, and acknowledge potential liability under the Oregon False Claims Act for false statements or fraudulent representations. Applicants anticipated to receive contracts valued at $150,000 or more must certify implementation of written policies preventing sexual harassment, sexual assault, and discrimination against protected class members prior to contract execution.
