About this file
This is a Responsibility Inquiry form (Attachment G) for the State of Oregon's Department of Human Services, Office of Self-Sufficiency Programs, Refugee Program, issued as part of Request for Grant Proposal S-10000-00016630 for Financial Assistance for HR1 Impacted Immigrants. The form serves as a pre-award evaluation tool to determine proposer responsibility and capability to perform contractual obligations. Proposers must demonstrate they possess appropriate financial, material, equipment, facility, and personnel resources necessary to meet all contractual responsibilities. The form requires documentation and responses regarding contract performance history, including the number of similar contracts completed within the last five years that remained within time and budget constraints with no claims, as well as any contracts that failed to meet those standards.
The Responsibility Inquiry requires proposers to disclose criminal or civil charges, indictments, or convictions within the last three years related to public contracts, antitrust violations, or fraud. Additionally, proposers must report any contract terminations for default, lawsuits from creditors or contract disputes, outstanding judgments, financial distress, bankruptcy filings, or reorganization actions. The form mandates disclosure of cash flow sufficiency to fund day-to-day operations throughout the proposed contract period and requires certification that the proposer possesses all required licenses, insurance, registrations, and is legally authorized to conduct business in Oregon. An authorized representative must sign and certify that all responses are complete, accurate, and not misleading. Failure to promptly provide requested information or clearly demonstrate responsibility may result in a finding of non-responsibility and rejection of the proposal.
