About this file

This is a Proposer Information and Certification Sheet attachment for Bid Solicitation S-10000-00015907 issued by the Oregon Department of Human Services (ODHS) for a Mobile Child-Caring Agency (CCA) Program. The solicitation seeks contractors to establish a mobile CCA program that provides supervision and support to children and young adults aged 5 through 20 who are in ODHS care and custody and currently without placement in temporary lodging settings. Proposers must complete this certification form as part of their bid submission, with proposals constituting firm offers valid for 180 days following the closing date. The document does not specify response deadlines, site visit requirements, bidder conferences, award dates, or contract terms in this attachment; those details are referenced as being contained in the RFP, the Sample Contract (Attachment H), and any issued addenda.

The certification sheet requires proposers to affirm compliance with multiple statutory and regulatory requirements, including Oregon tax law compliance, non-discrimination in employment and subcontracting practices, equal wage payment requirements under ORS 652.220, and policies preventing sexual harassment and discrimination (required for contracts with anticipated prices of $150,000 or more). Proposers must also certify they are not listed on federal exclusion lists maintained by the Office of Foreign Assets Control or the System for Award Management, that no conflicts of interest exist, and that all proposal contents are truthful and accurate. The document does not contain pricing information, set-asides, incumbent contractor details, or funding amounts in this attachment.
