About this file

This document is an Attachment C Certified Disadvantaged Business Outreach Plan for a Request for Proposal (RFA) by the Department of Administrative Services (DAS) Procurement Services, specifically for CW Behavioral Health – Treatment Foster Care Services under Bid Solicitation S-10000-00013544. The form is designed to encourage participation of certified disadvantaged businesses in state-funded contracts, with a focus on minority-owned, woman-owned, service-disabled veteran-owned, and emerging small businesses certified under Oregon Revised Statute 200.055 by the Oregon Certification Office for Business Inclusion and Diversity (COBID).

The document requires proposers to disclose their certification status, provide lists of certified and potentially disadvantaged firms they have worked with in the past two years, and outline their plans for subcontracting opportunities. Proposers must describe steps they will take to solicit certified firms, detail potential subcontracting opportunities and their approximate dollar values, and indicate their willingness to report subcontractor identities and contract values to COBID if awarded the contract. The form emphasizes that the information submitted will not be considered in any scored evaluation, but demonstrates the state's commitment to providing equal opportunities for diverse businesses in procurement processes.
