About this file
This document is an Applicant Information and Certification Sheet for Request for Application (RFA) Bid Solicitation S-10000-00013544 issued by Oregon Department of Human Services (ODHS) for CW Behavioral Health Treatment Foster Care Services. The document is a standardized form requiring potential bidders to complete and certify various legal and operational compliance statements in order to be considered for the contract, with applicants required to verify they are either currently performing or new to Behavioral Health Treatment Foster Care (BH-TFC) Services. The application represents a firm offer valid for 180 days and requires the applicant to agree to perform services according to the negotiated scope of work and performance standards.
The certification sheet includes numerous compliance requirements covering non-discrimination practices, tax law adherence, wage equity, conflict of interest disclosures, and regulatory compliance across multiple Oregon Revised Statutes (ORS). Key certifications include agreeing not to discriminate in employment practices, complying with wage and compensation regulations, confirming the applicant is not on government exclusion lists, and verifying that all application contents are truthful and prepared independently. The document also highlights that any false statements could result in potential claims under the Oregon False Claims Act, with associated liabilities and penalties.
