About this file
This document is an Attachment D - Responsibility Inquiry form from Bid Solicitation S-10000-00011070 for the ODHS (Oregon Department of Human Services) Child Welfare Independent Living Program (ILP) Alternate Provider Lists. The form is used by ODHS to determine the responsibility of an Applicant prior to awarding and executing a contract. It requires the Applicant to provide information about their financial, material, equipment, facility, personnel resources and expertise, as well as their past contract performance, legal issues, financial condition, and licensing/authorization to do business in Oregon.
The related state and local contract opportunity is a Request for Applications to create lists of Alternate ILP Providers in all ODHS District/Service Areas. This will allow ODHS to immediately begin working with providers on the list to become qualified ILP Service Providers if additional services are needed within a District. The contract opportunity involves the state of Oregon, specifically Multnomah County.
