About this file
This document is an Attachment A from Bid Solicitation S-10000-00011070 issued by the Oregon Department of Human Services (ODHS) for an Alternate ILP Provider List for Child Welfare Independent Living Program (ILP) services. The RFA seeks to establish lists of Alternate ILP Providers in all ODHS District/Service Areas that can be utilized if new or additional ILP services are needed within a District. Applicants can apply to be included on the list for one or more of the 16 ODHS districts, and if awarded a contract, will be expected to perform the scope of work and meet the performance standards set forth in the final negotiated contract. Applications for the Alternate ILP Provider List will remain on the list for a maximum of five years unless the applicant requests to be removed earlier.
The document outlines the required certifications and representations applicants must make, including compliance with tax laws, non-discrimination policies, and disclosure of conflicts of interest. Applicants must also certify they are not on federal exclusion lists and that the contents of their application are truthful and accurate. The RFA indicates ODHS may not enter into a contract with an anticipated contract price of $150,000 or more with an applicant that does not have a written policy and practice of preventing sexual harassment, sexual assault, and discrimination against protected classes.
