About this file
This document is an Attachment D - Employee Qualifications Checklist for a Request for Application (RFA) issued by the State of Oregon, acting through the Oregon Department of Human Services (ODHS) and its Office of Vocational Rehabilitation (VR), to solicit applications from providers qualified to perform vocational rehabilitation job placement services. The RFA is open to businesses, non-profit organizations, and self-employed individuals who meet the experience, education, or training requirements to provide employment support services to persons with disabilities. Contractors must submit a completed Attachment D for each employee who will provide job placement services, confirming their qualifications, including attending required job placement services contract training and core competency modules, meeting experience or education requirements for specific participant tracks, and submitting supporting documentation such as criminal background checks, training certificates, and resumes.
The pricing terms, set-asides, and funding sources for this contract opportunity are not detailed in this attachment. However, the RFA specifies that the contract will be for vocational rehabilitation job placement services to VR participants across three different service tracks, with specific qualifications required for providers of the higher-level Track 2 and Track 3 services.
