About this file
This document is an Attachment A to a Request for Application (RFA) issued by the Oregon Department of Human Services (ODHS) and its Office of Vocational Rehabilitation (VR) for Vocational Rehabilitation Job Placement Services. The RFA is seeking applications from qualified providers, including businesses, non-profit organizations, and self-employed individuals, to provide employment support services to VR participants. The Application requires the applicant to agree to complete a background check, acknowledge the RFA requirements and contract terms, certify compliance with certain laws and regulations, and affirm the truthfulness and accuracy of the application.
The RFA has an anticipated contract term, but does not specify the length. Pricing terms, set-asides, incumbent providers, funding sources, and award amounts are also not detailed in this attachment. The key focus of the RFA is to identify qualified providers to deliver vocational rehabilitation and job placement services to individuals with disabilities in the state of Oregon.
