About this file
This document is Attachment B to a Request for Proposals (RFP) issued by the Oregon Department of Human Services (ODHS) for Outreach, Referral, and Language Services to Multicultural Populations with Disabilities. The RFP seeks proposals to provide outreach, language services, referral, training, and employment services to multicultural populations with disabilities across various Oregon counties. Proposers must identify key personnel and specify which qualification criteria they meet. The proposal is a firm offer valid for 180 days, and the selected contractor must agree to the contract terms and conditions.
The RFP requires proposers to certify compliance with various state laws and regulations, including non-discrimination in employment, preventing sexual harassment, and avoiding conflicts of interest. Proposers must also confirm they are not on any government exclusion lists. Pricing terms and funding sources are not detailed in this attachment, but the RFP indicates the anticipated contract value is $150,000 or more.
