About this file
This document is an Addendum #1 for Gwinnett County's Request for Proposal RP037-23 for the Provision of Pre-Approved Non-Emergency Transportation of Gwinnett County Residents on an Annual Contract. The proposal deadline has been rescheduled to Friday, October 27, 2023 at 2:50 PM. The County is seeking proposals for two separate sections - Section A: Senior Programs and Section B: Shuttle Services. For Section A, the County estimates approximately 15,000 ambulatory trips and 3,700 wheelchair trips annually. Section B includes a new shuttle service to the Veteran Affairs Hospital in Atlanta. Providers must submit separate proposals and cost proposals for each section.
The current provider is AllStars Transport & Logistic, LLC. The County has approved funding for these services but requests that contractors provide their most competitive pricing. No bonds are required. The County will award a primary and secondary provider for Section A to ensure all client needs are met. The provider will be responsible for supplying the fleet and all resources needed to perform the transportation services, while the County will provide support with daily manifests, communication, and funding.

ADDENDUM #1
RP037-23, Provision of Pre-Approved Non-Emergency Transportation of Gwinnett County Residents on an
Annual Contract
**THE PROPOSAL DEADLINE FOR RP037-23 HAS BEEN RESCHEDULED FOR FRIDAY, OCTOBER 27, 2023 AT
2:50 PM.**
REVISIONS:
R1. One unbound single-sided original, five (5) exact copies, and one electronic pdf copy on flash drive should be submitted.
QUESTIONS & ANSWERS:
Q1. How much is currently being budgeted for these services for each section, over the potential duration of this contract?
A1. The County has approved funding for the services requested under this contract. However, the County requests that contractors provide their most competitive pricing for their proposals.
Q2. Will there be an increase to this budget, should the demand increase?
A2. The County has approved funding for the services requested under this contract. Any increases will be determined at the time of need.
Q3. Who is the current provider, operating the service?
A3. The currently contracted provider for these services is AllStars Transport & Logistic, LLC.
Q4. How many total trips were completed last year?
A4. Approximately 18,770 trips were completed in the last year for services listed in Section A. Services for
Section B yielded under 100 trips, however, this is not an accurate estimate for this contract, as the warming station programs and shuttle service were not yet in place.
Q5. How many ambulatory, wheelchair and stretcher trips are currently being completed daily?
A5. The daily average, Monday – Friday, is 73 ambulatory trips, 14 wheelchair trips, and zero stretcher. The daily average, Saturday and Sunday, is 18 ambulatory trips, 3 wheelchair trips, and zero stretcher.
Q6. How many vehicles are currently in the fleet? How many of those vehicles are ambulatory only, wheelchair accessible and stretcher accessible? Does the current fleet size suffice? Or is there a need for additional vehicles?
A6. The County does not own a fleet of vehicles for this transportation service. The selected provider will be responsible for the provision of the fleet that is used to perform the service. Respondents should demonstrate possession of a minimum of six vehicles and drivers must be able to manage daily trip requests as needed. Based on past data, a fleet of six (6) to ten (10) vehicles is recommended to manage this contract. A minimum of three (3) wheelchair accessible vehicles is also recommended. These are estimates and are subject to change based on demand.
Q7. Does the County provide any resources or facilities to provide this service or is it the County's expectation that the contractor provides all resources needed to provide this service?
A7. The selected provider will be responsible for the provision of the fleet that is used to perform the transportation service. Storage of the fleet and routine maintenance are the responsibility of the provider.
The resources that the County provides include daily manifests, support with communication and funding.
Q8. What improvements, as it relates to how the service is being provided, would the County like to see?
A8. The County requests that the awardee provides services in line with the contracted terms. If all provisions and the scope are followed, then services should run smoothly and to the Gwinnett Standard.
Q9. The RFP states that if you bid on Section A and Section B, the proposal should be separate. Are the
Section A and B proposals submitted in different envelopes, as well, and on a separate flash drive, too?
A9. Proposals for Section A and Section B must be submitted separately in two separate envelopes, with the envelope clearly stating what section each submittal is for. A separate Cost Proposal and flash drive must be submitted for each section.
Q10. The RFP states that the start date of the contract is December 11, 2023. What is the date vendors will be notified of the award, and how many weeks are allowed for the implementation of the program? Vendors need to hire additional drivers, perhaps procure additional vehicles, conduct training on County policies, and learn each other's systems, if necessary.
A10. The award date will be determined at the completion of the scoring process. Gwinnett County will make every effort to issue an intent to award letter to the selected service providers so that they can prepare to provide the requested services.
Q11. If subcontractors are utilized, is it required to submit all the required information for providers for each subcontractor?
A11. Subcontractor information should be included on the List of Subcontractors form on page 23 of the RFP documents. Subcontractors must be approved by the user department prior to providing services. Only the submitting contractors will be required to submit the required RFP forms.
Q12. Is a bid, performance, or payment bond required for this bid?
A12. Bonds are not required for this solicitation.
Q13. Standard Insurance Requirements: Under Commercial General Liability Insurance, it indicates that under
Commercial General Liability additional coverage must apply. It appears that the highlighted coverages are not relative to an NEMT contract when comparing to state NEMT broker programs. Can you please review these requirements? The clients will not enter or be present at the provider’s facility/location, Incidental Malpractice is not relative, nor is Hostile Fire Pollution Wording.:
o 1986 (or later) ISO Commercial General Liability Form o Dedicated Limits per Project Site or Location (CG 25 03 or CG 25 04) o Additional Insured Endorsement (Form B CG 20 10 with a modification for completed operations or a separate endorsement covering Completed Operations) Blanket Contractual Liability o Broad Form Property Damage o Severability of Interest o Underground, explosion, and collapse coverage o Personal Injury (deleting both contractual and employee exclusions) o Incidental Medical Malpractice o Hostile Fire Pollution Wording
A13. Incidental Medical Malpractice and Hostile Fire Pollution Wording may be removed from the insurance requirements.
Q14. How many vehicles are estimated for Section A operations?
A14. Based on past data, a fleet of six (6) to ten (10) vehicles is recommended to manage Section A of this contract. A minimum of three (3) wheelchair accessible vehicles is also recommended. These are estimates and are subject to change based on demand.
Q15. For Section A, Standard Hours are from Monday - Sunday from 6:00 am - 6:00 pm, and trips outside of standard hours must be provided. Is this a 24/7 service? Or do hours stop and start at certain times?
A15. The selected provider is not expected to operate 24/7. Most of the trips that clients need occur between the hours of 6:00 am and 6:00 pm, however, there are occasions when clients need to travel outside of that time frame. When approved by the County, the selected provider would be requested to accommodate those trips.
Q16. For Section A, 3. Transport typically occurs from the client’s home to a medical appointment, dialysis treatment, place of employment, pharmacy, social service agency, senior center, place of worship, or grocery store, and a return trip back home, with passengers contacting the provider in advance to arrange a ride to a location within the provider’s service area. How does the provider confirm that the trips are for these particular places?
A16. The County works directly with our seniors to schedule their trips. The County will provide a manifest to the selected providers which includes the address of the pickup and drop off locations. Per our policy, the trips cannot be changed the day of service.
Q17. For Section A, how does the vendor confirm that the client is eligible for this program?
A17. An assessment process is completed in the County to ensure client eligibility for the program. The selected provider will not have to determine eligibility.
Q18. Under Special Provisions, 2. Some clients may require an escort to assist them on the journey. Is the provider required to provide an escort if needed? Is the escort from the facility or a family member? Due to the space in the vehicle, is the benefit limited to one escort?
A18. If the client wishes to bring an escort, they must bring their own escort. The provider is not required to escort the client past the door of their destination. It is rare for someone to bring more than one escort, however, a maximum of two (2) escorts is allowed. This would be indicated on the manifest.
Q19. The Section A Cost Proposal form shows for Ambulatory Clients 91,000 miles annually and for
Wheelchair 34,000 miles annually. Can you please provide the number of trips annually for each?
A19. All figures are based on annual estimates. The breakdown would be approximately 15,000 ambulatory trips and 3,700 wheelchair trips.
Q20. At times, clients may have steps leading into their residences. Are providers expected to transport clients upstairs? If so, what is the stair limit? And is this information provided when the trip information is sent to the provider?
A20. Service is door through door. This means that drivers must be prepared to walk a client from the door of their home to the vehicle and then walk them to the door of their destination and ensure their return home.
Drivers are expected to supervise a client walking up and down stairs to ensure their safety. However, drivers are not expected to help with any type of lifting. If a client requires significant help up and down stairs, they would be expected to bring an escort with them on all trips.
Q21. How many stretcher trips are estimated?
A21. Currently, the County does not have clients that utilize stretcher service. However, the selected provider must have the capability to accommodate this type of trip, if requested.
Q22. Can you please describe the shuttle operations? Are there set times that the shuttle has to pick up clients? For example, every hour or every two hours? Also, is the shuttle currently in operation? If so, how many vehicles are being used at this time?
A22. The shuttle operation is a new service we are providing and is not currently in operation. We intend for at least two round trips minimum to take place with the shuttle daily. Providers should state their capacity for the number of round trips that they can provide daily. The County will work with the awarded vendor upon award for time and capacity. A24, A60 provide more detail if necessary.
Q23. What time schedule will the shuttle run? Does the shuttle run on Saturday and Sunday? How often should the shuttle pick up – what is the shuttle schedule? For each location. How long can the client wait for the shuttle?
A23. The time schedule will be a morning trip and mid-afternoon/afternoon round trip to the Veteran Affairs site in Atlanta for Veterans only through the OneStop 4 Help Veteran and Family Services. The County will schedule with the awarded provider and residents. The County does not currently anticipate operating the shuttle on Saturday or Sunday. This is veteran-specific through the OneStop 4 Help program. A22 and A24 provides more detail as does A60.
Q24. Does the shuttle drop off clients to a centralized location?
A24. Please refer to Section II: Scope of Work, Section B where the detail is included on the shuttle. The County is seeking transport to travel to the Veteran Affairs Hospital in Atlanta from the Veteran and Family Services Center in Lawrenceville. Address Origin: VFS Center, 567 Swanson Drive, Lawrenceville, GA.
Address Destination: Joseph Maxwell Cleland Atlanta VA Medical Center, 1670 Clairmont Road, Decatur, GA 30033-4004.
Q25. Is the shuttle required to transport wheelchair clients and ambulatory clients? What type of vehicle is required?
A25. The vendor should be able to transport those who are ambulatory with an option for wheelchair clients.
The vehicle type is based on that answer.
Q26. Section B – Shuttle Services: Does the number of trips Monday – Friday (20 trips) and Saturday/Sunday
(0-10) trips include the shuttle passengers? Are the 20 trips/0-10 trips one-way or round trip?
A26. The shuttle is separate from the estimated trips listed for Monday through Friday (20) and the
Saturday/Sunday (0-10). The trips are considered Round Trip or the shuttle.
Q27. Section B – Shuttle Services: What number of trips equate to 20 clients per day? Is this from an annual trip count for this program?
A27. The trips are based on need and quantity currently transported. The shuttle service is separate from these trips. Please also see the response to Q22.
Q28. Operating Standards: The RFP states, Pick-up and delivery shall be on-time as scheduled except for extenuating circumstances, which must be communicated to Gwinnett County in writing via email and followed up with a telephone conversation (not a voicemail message) as soon as circumstances allow.
Pick-up and drop- off can be up to one hour before and one hour after scheduled appointment time. Can you please explain “….and one hour after scheduled appointment time?”
A28. The provider can pick up a client up to an hour before the client’s appointment time. (Example: If the client has a 10AM appointment, the provider can arrive as early as 9AM to pick up the client). When the client finishes their appointment, they have the option to call to let the provider know that they are ready to be picked up or they will have a set pick up time. The provider will then have up to an hour after the client calls them to be picked up to return to bring them home. The hour is a window of flexibility to accommodate for traffic. Where possible, clients should be picked up in under an hour.
Q29. Operating Standards: The RFP states that the Driver/Provider shall immediately notify the client and
Gwinnett County staff in the event of unavoidable delays. What are the hours of Gwinnett County staff?
A29. The Gwinnett County staff assigned to this project work Monday through Friday, 8:00 am- 5:00 pm. There is a dedicated transportation phone line that accepts messages at any time. Additionally, management level staff have access to email and phones outside of normal business hours and can be reached in the event of an emergency.
Q30. The RFP states it is the intent of Gwinnett County to award Section A of the contract to a Primary Service provider and a Secondary Service Provider so that all client needs are met. However, the County reserves the right to award the contract in its best interest. The Primary Service Provider will be the first company to be advised of a client. The County reserves the right to alternate clients to both service providers considering the total number of existing clients and other applicable information to determine when to utilize the Secondary Service Provider.
If the County alternates from the Primary Service Provider to Secondary Service Provider, at what point is a trip given to a secondary provider for Section A?
A30. Trips will be given to the primary provider until the primary provider notifies the transportation team that they can no longer accommodate any new clients. At that time, the secondary provider would be given trips. It is the County’s intention that once a client is assigned to a primary or secondary provider, they will remain with that provider for the duration of the time they ride.
Q31. What is the budget for this project?
A31. The County has approved funding for the services requested under this contract. However, the County requests that contractors provide their most competitive pricing for their proposals.
Q32. Who is the incumbent software provider? What were the issues the Agency was facing with the incumbent SW provider, if any?
A32. Software is not provided by the County. The provider will be responsible for any software necessary for their operations.
Q33. Can the Agency please share the ridership data for the past year?
A33. The estimates provided in the RFP reflect the current data for the past year.
Q34. How many vehicles were operational under Section B of the scope of work of this RFP? Also, how many vehicles are kept on standby during operational hours?
A34. The contracted vendor maintained a fleet of five (5) vehicles. The selected service providers will be responsible for the number of vehicles kept on standby.
Q35. Is there a requirement for a rider application?
Q35. Depending on the program that the client is enrolled in there are often age or disability requirements that clients must meet. Income eligibility is also a factor. The selected respondent will not be responsible for determining eligibility.
Q36. Is data migration a requirement from the old vendor's portal and application or is an entire new set-up required?
A36. Data migration is not a requirement. The selected vendor should provide a portal and application for management of the transportation services. There is no portal or application with the County or previous vendors.
Q37. Please confirm whether the Agency expects the bidder to provide dispatching and scheduling software?
If yes, please provide details about the software requirements.
A37. The respondent may use whatever software or devices they need to meet the standards of the contract specifications.
Q38. Does the vendor need to provide any technical support as well as training for this project? Is the Agency expecting repeated training every year or one-time training during the project set-up phase?
A38. The selected vendor is expected to ensure that all drivers are trained (general orientation, consumer service, courtesy, and sensitivity awareness, certified defensive driving, vehicle orientation and pre-trip inspections, proper handling and securing of mobility aids and basic first aid and CPR). Continuing education will be required to ensure continued compliance in this area. Please refer to the Department of Human Services’ Transportation Manual. This manual is available online at https://odis.dhs.ga.gov/General/Home/DhsManuals?id=843
Q39. Is there any hardware/MDT’s required for the vehicles? If yes, is the agency willing to buy MTDs from the vendor or will it procure the MDTs separately itself?
A39. There is no hardware/MDT’s required for the vehicles. The respondent can use any necessary tools needed to meet the contract standards.
Q40. Can the bidder submit a detailed cost proposal along with the cost proposal format included in the solicitation document?
A40. Pricing must be submitted for only the line items on the provided Cost Proposal forms.
https://odis.dhs.ga.gov/General/Home/DhsManuals?id=843
Q41. Does the vendor have to provide an insurance certificate with the proposal?
A41. A certificate of insurance is not required with the proposal submittal. The awarded contractors will be required to submit a certificate of insurance to meet the insurance requirements listed in the RFP prior to beginning services.
Q42. Will the Agency accept electronic signatures on the forms and cover letter?
A42. Yes, electronic signatures are acceptable. However, hardcopies of the proposals must be submitted to the
Gwinnett County Financial Services – Purchasing Division.
Q43. Will the Agency accept an electronic submission for this proposal?
A43. Gwinnett County cannot accept proposals digitally. Proposals must be submitted to the Gwinnett County
Financial Services – Purchasing Division – 2nd Floor, 75 Langley Drive, Lawrenceville, Georgia 30046.
Q44. Is there a page limit for the technical proposal?
A44. There is no page limit for the technical proposal.
Q45. In order to allow bidders to prepare a more detailed and informed response, can the Agency please extend the submission deadline for the proposals?
A45. The proposal deadline has been rescheduled for Friday, October 27, 2023 at 2:50 PM. Proposals must be submitted at the Gwinnett County Financial Services - Purchasing Division – 2nd Floor, 75 Langley Drive, Lawrenceville, Georgia 30046.
Q46. Section A: The Cost Proposal sheet allows for the mileage you provide and the Unit Price. Would the
County not consider a base trip price (that includes a set amount of miles) plus a fare for mileage? Similar to: Total Cost = (Rate per Mile * Total Miles) + (Base Pay per Customer * Number of Customers)
A46. At this time, the cost structure is per the listed chart in the proposal document. The Cost Proposal will not be revised.
Q47. How many wheelchair customers will travel a distance of 34,000 miles per year?
A47. The total number of wheelchair clients varies. The estimated 34,000 miles is an annual estimate for the total number of miles to be travelled by all wheelchair clients combined.
Q48. Section A: What is the ratio of customers who have entrance stairs?
A48. The County does not maintain this information.
Q49. Section A: Do you have information about the number of bariatric wheelchairs or stretchers available?
A49. The County does not have the information on the number of bariatric wheelchairs. The County does not have any stretchers at this time.
Q50. Do you have information regarding the quantity of bariatric wheelchairs or stretchers, as well as their associated weight and height specifications?
A50. The County does not have the information on the number of bariatric wheelchairs. The County does not have any stretchers clients. The County does not maintain information on height and weight specifications.
Q51. What is the mileage for out-of-county travel?
A51. The County does not have an estimated mileage for out-of-county trips. The request for proposals requests a flat fee for out-of-county trips.
Q52. How many stretcher customers will travel a distance of 50 miles per year?
A52. The County does not currently have any stretcher clients. The estimated 50 miles is an annual estimate for the total distance to be travelled by potential stretcher clients.
Q53. To determine if we're eligible to respond, can you clarify the demonstration of ownership? We're a transportation network that has significant vehicles and drivers contracted to us but not direct ownership.
A53. The selected provider must be able to submit vehicle maintenance and service records and insurance coverage for all vehicles used in the performance of the scope of work for Section A to the County.
Vehicles must be available for inspection by the County prior to use. All required documentation for drivers must be provided as well (MVR, driver license, drug screening results, proof of training, finger printing and background checks (see pages 5-6 of RFP). If all these documents can be provided, then ownership of the vehicles is not required. Please refer to the Department of Human Services’ Transportation Manual.
This manual is available online at https://odis.dhs.ga.gov/General/Home/DhsManuals?id=843.
Q54. I may have missed it in the paperwork. Can you explain how we are supposed to include the information for the proposal evaluation and selection procedures process? It is not listed with the information that should be returned with the proposal.
A54. The proposal evaluation and selection procedures section explains the County’s process for the evaluation of proposal submittals as well as the scoring criteria that will be used to evaluate proposals. Proposal submittals should demonstrate the provider’s understanding of the contract and its ability to meet or exceed the requirements. The method of presentation shall be up to the proposing firm.
Q55. There is a Section A and Section B. Do we have to submit a proposal for both. If not, do we need to submit pg 36 for no bid for the section we are not submitting a proposal for section?
A55. Providers that are interested in both sections should submit a separate proposal for each section. If submitting a proposal for both sections, the No Bid form on page 36 will not be required.
Q56. We have to submit the cost proposal separately. Do we need to put the proposal and cost proposal in separate envelopes and then into one bigger envelope to keep everything together?
A56. The technical proposal and cost proposal must be submitted in separate sealed envelopes. Vendors may submit their technical proposal and cost proposal in the same container, as long as the technical proposal and cost proposal are in separate sealed envelopes.
Q57. Do we need to submit a copy of our occupational tax certificate and COI with the proposal?
A57. No, however, the awarded contractors will be required to submit a certificate of insurance to meet the insurance requirements listed in the RFP prior to beginning services.
Q58. What if we plan on using a subcontractor but do not have the subcontractor we would use yet? Can we submit that we will be using subcontractors and provide the subcontractor's information at a later date before we start using them?
A58. Yes. Any subcontractor must be approved by the user department prior to providing services.
Q59. On page 19, there is no separate line item for wheelchairs. Is this service not available for wheelchair clients?
A59. There is no wheelchair requirement for Section B of the RFP.
Q60. On Section B Cost Proposal, Line #7 Shuttle Service. Can we get additional information on this? Would the Provider be expected to the Atlanta VA in the morning to drop off clients and back in the evening to pick them up or will it be a constant back and further between the Lawrenceville and Atlanta locations throughout the day?
A60. Please see A22: The shuttle operation is a new service we are providing and is not currently in operation.
We intend for at least two round trips minimum to take place with the shuttle daily. Providers should state their capacity for the number of round trips that they can provide daily. The County will work with the awarded vendor upon award for time and capacity. A24, A60 provide more detail if necessary.
https://odis.dhs.ga.gov/General/Home/DhsManuals?id=843
Q61. On Section B Cost Proposal, Line #7 Shuttle Service. There are 780 trips listed. Is one trip roundtrip or one way?
A61. All trip estimates are for roundtrip service.
Q62. Section B Cost Proposal: The information in Section B does not mention if the service requested is door-through-door or curbside. Please clarify.
A62. Door-through-door service is required for the services in Section B.
Q63. It was mentioned that for section A of the proposal, the county anticipates to award two contractors, a primary and a secondary. Is it possible that the county only awards one contractor for section A, if they possess the capital to run the operation completely?
A63. The County will award a primary and secondary as stated in the proposal. The primary should be able to run the operation to its full requirement. If the primary is able to keep up and complete all operations, per the contract, then the secondary would not be needed.
Q64. For the pricing structure, if wheelchair trips are required, will a load rate be acceptable or will it be priced exclusively per mile?
A64. A load rate will not be acceptable. Wheelchair trips are standard in daily travel, and it will be paid exclusively to the price per mile.
Q65. On page 38, section II. Delivery - Can someone explain this section? We do not quite understand what is meant in line items A or B.
A65. This language is from the County’s General Instructions for Vendors, Terms and Conditions, and does not apply to the services under this contract. The awarded service providers will coordinate services with the Department of Community Services Health and Human Services division.
Q66. The RFP talks about providing services on Holidays for Section A. We would like to request that transportation on Thanksgiving Day and Christmas Day not be included or expected. Doctor's offices nor dialysis centers are open on these two days.
A66. The County’s transportation service is 365 days a year. All the holidays listed in the proposal are valid for travel.
Q67. On page 4, section 4, the RFP talks about standard service hours. Traditional stand service hours for the
NEMT Industry are normally Monday through Friday from 6 a.m. to 6 p.m. and all day on Saturdays and Sundays. We would request that all-day Sundays be included as non-standard service hours. There is an increased overhead cost to operate for the small amount of clients that actually travel on this day.
A67. Your RFP response should include what you are able to provide for service. The County has provided the hours, days, and time within the proposal documents.
Q68. Section A - Senior Programs does not talk about some other services that are expected to be provided while transporting clients. One of them is helping clients gather, load, and unload groceries or other items obtained while out on transportation trips. We would request to include a grocery delivery fee of $25 for any transportation trips that drivers have to load and unload clients' groceries from the grocery store or similar type locations and into the client's home.
A68. Grocery delivery fees will not be covered. It is not a part of the scope of work for drivers to load and unload a client’s groceries. If a client needs this level of support, they will be encouraged to bring an escort who can assist. In this situation, the County would pay for the escort.
Q69. Section A - Senior Programs does not talk about oversized wheelchairs. Keeping with traditional NEMT standards, we would like to request that an oversized wheelchair load fee be included for clients who have oversized wheelchairs or our over 250 pounds.
A69. An oversized wheelchair load fee is not listed within the scope of this solicitation and will not be considered.
Q70. On page 8, Section F: Operating Standards #5. The provider is requested to call clients the day before, and at least 10 minutes before they reach the client. Asking for the Provider to wait 10 minutes is too long in our opinion. We request this time be reduced to 5 minutes. With the large area of Gwinnett County and the number of clients that need to be served, every minute truly counts.
A70. The operating standards were developed to ensure consistent delivery of service to our clients. The ten-minute wait time is a component of our policies and procedures and included in our operating standards.
Q71. For Section A, there are situations where transportation cannot be provided via the Gwinnett County
Programs. Often clients will request for the Provider to provide transportation outside of Gwinnett County program guidelines. We generally do not like to provide private transportation outside of the guidelines of the county's programs. Could the Health and Human Services department add something to the contract regarding their stand on this matter?
A71. Private transportation outside of the guidelines of the County’s programs is not associated with this non-emergency medical transportation solicitation and will not be referenced in the contract. The Health & Human Services Division will not require the provision of private transportation.
Q72. Can a provider be awarded both sections or is it one or the other?
A72. Yes, it is possible for a responder to be awarded one or both sections.
Q73. Are RFPs required to be submitted with a flash drive?
A73. Yes, a flash drive or other electronic device containing an electronic version of the proposal submittal should be submitted with your proposal.
Q74. Does each section require all the same forms be submitted separately or does one set of forms suffice for both sections?
A74. Yes, all required forms should be submitted for each section.
Q75. Can you please help us to understand the proposal submission requirements. Is it 4 total written proposals and 1 electronic identical proposal (flash drive)? But the Cost Proposals will need to be 4 separate written proposals?
A75. If the Respondent would like to be considered for both Part A and Part B, you must provide two (2) separate proposals, one for each section. Each proposal package must include: one unbound original narrative, five
(5) bound copies, a flash drive with an electronic copy of the proposal and a sealed Cost Proposal. If the Respondent would like to be considered for only one part, the proposal package must include: one unbound original narrative, five (5) bound copies, a flash drive with an electronic copy of the proposal and a sealed Cost Proposal. Only one copy of the Cost Proposal is required per section.
Q76. REQUIRED CONTRACT PROVISIONS FOR NON-FEDERAL ENTITY CONTRACTS UNDER FEDERAL
AWARDS: Are we required to send proof of our UEI number from SAM.gov? If so, what is considered sufficient proof?
A76. The REQUIRED CONTRACT PROVISIONS FOR NON-FEDERAL ENTITY CONTRACTS UNDER FEDERAL AWARDS forms on pages 26-33 of the RFP documents should be completed and submitted with your proposal. Gwinnett County must be able to verify that submitting firms have been assigned a UEI number and/or that they have an active registration status with SAM.gov.
Q77. Can Gwinnett County be one of our references, since we currently transport on a temporary contract?
A77. Contractors may use Gwinnett County as a reference.
Q78. Are you able to share with us the NEMT incumbent, the previous contract number, and the awarded amount?
A78. The currently contracted provider for non-emergency medical transportation services is AllStars Transport
& Logistic, LLC. These services were awarded as part of BL101-22 (Provision of Pre-Approved Non- Emergency Medical Transportation of Seniors and Disabled Adults on an Annual Contract), which was awarded for $460,081.00.
This addendum should be signed in the space provided below and returned with your bid. Failure to do so may result in your bid being deemed non-responsive.
