14. DESCRIPTION OF AMENDMENT/MODIFICATION (Organized by UCF section headings, including solicitation/contract subject matter where feasible.)
This amendment changes the Statement of Work from "Community Treatment Services, Statement of Work, January 2017" to "Community Treatment Services, Statement of Work, April 2017. Specifically, the following sections have been replaced in their entirety: Sections 2.2-2.2.9 and 3.3-3.4.
The closing date of this solicitation has also been amended to 5/15/2017 at 2:00 p.m. Eastern.
Except as provided herein, all terms and conditions of the document referenced in Item 9A or 10A, as heretofore changed, remains unchanged and in full force and effect.

2.1 CONTINUATION OF SF-1449 BLOCK 20
SCHEDULE OF SUPPLIES/SERVICES
The contractor shall be capable of providing outpatient substance use, mental health, and sex offender treatment services to offenders confined in community-based programs. The contractor shall provide a physical location from where all required services can be provided. The location where services are to be provided should be within a ten (10) mile radius of City Hall, 241 West South St.
Kalamazoo, MI 49007 and should be accessible to local public transportation.

All references to Residential Re-Entry Center (RRC) and Community Correctional Center (CCC) shall be treated equally for the pur-pose of this contract.
The anticipated total contract period is 10 years (to begin upon the effective date of contract award). The anticipated performance start date is July 1, 2017.
DECISION RULE CRITERIA
OPERATIONS:
1. Quoter must demonstrate adherence to Cognitive Behavioral Therapy (CBT) or a compatible theoretical model.
• Quoter may submit a mission statement or treatment philosophy to satisfy this criterion.
• Quoter must clearly explain how any additional treatment protocols will be used and how the additional treatment proto-cols are compatible with CBT.
• Note any affiliation with ATSA
2. Quoter must submit a plan for addressing Crisis Intervention Assessments required to be completed within 24 hours of request by CTS staff. Quoter must provide crisis intervention procedures for emergency referral and evaluation of inmates if the inmate cannot be evaluated by the agency’s mental health provider.
3. Quoter must submit a plan that details how treatment files for Bureau inmates shall be maintained.
• Quoter must detail how Bureau inmate files will be maintained separately from non-Bureau files, kept on-site, and se-cured.
• Any intended use of electronic database systems must be detailed and address the electronic system’s ability to meet the Bureau’s electronic security requirements. All persons with access to Bureau files on the electronic system must be identi-fied and included on the Technical Personnel document.
4. Quoter must submit a plan to provide treatment services to non-English speaking inmates as necessary.
5. Quoter must have a comprehensive inmate accountability program to be used in conjunction with the Bureau provided Treatment Services and Accountability Log that ensures every inmate is accounted for upon arrival, during treatment, and departure from the proposed performance site.
FACILITY:
6. Quoter must demonstrate that the proposed performance site is within the geographic restrictions, will be available on the contract effective date, able to accommodate the volume of treatment services required in the Statement of Work, and in compliance with Americans with Disabilities Act (ADA) requirements.
• Quoter must submit a map (e.g. MapQuest, Google, etc.) that illustrates that the proposed performance site(s) is within the geographic restrictions.
• Quoter must submit a deed, lease, bill of sale, options to lease, or option to buy in the name of the Quoter. If the proposed performance site is at an Residential Reentry Center, the Quoter must submit a statement that services will be provided in a space dedicated to treatment. All documentation submitted must show the proposed performance site will be available on the contract effective date.
• Quoter must submit documentation (e.g., written summary, pictures) that describes or illustrates the performance site lay-out, number of rooms, size of rooms.
• Quoter must address site compliance with ADA, or a plan for ADA accommodation to include restroom accommodations.
• Quoter shall provide the names and services of businesses in shared space or within close proximity of the same space.
7. Quoter must submit State and local licensing statutes to operate treatment facility for substance use/mental health/and sex offeder treatment, and provide the necessary license to satisfy the requirements.
• Documents must be in the name of the Prime Contractor, sub-Contractor licensing is not acceptable.
• If the quoter is in the process of obtaining a valid license, they may submit the completed application, provisional license, official email correspondence, certified letter, business license and/or other official documentation.
• If State or local law does not require facility licensing, the quoter must provide the statute that this factor does not apply, and submit official documents that allow services to be performed.
STAFFING:
8. Quoter must complete and sign the Technical Personnel document attached to the solicitation posting.
• Quoter must identify the position and responsibility of all personnel proposed to meet the SOW requirements, to include all employees, subcontractors, volunteers, and interns who have face-to-face interaction with inmates or who have access to their personally identifiable information, treatment records, contract documentation, billing invoices, or any other inmate information.
• All authorized negotiators must be identified on this document.
9. Quoter must ensure all personnel providing direct clinical services to Bureau inmates meet the experiential, educational and licens-ing requirements outlined in the SOW with appropriate licensure/certification to practice independently (i.e., without direct supervi-sion) as required by the state authority where the services are being provided.
• Quoter must have a qualified practitioner with prescriptive authority meeting the standards of practice established by his/

her State’s professional regulatory board to provide medication monitoring and psychiatric evaluations.
• Resumes must be submitted for all personnel providing direct clinical services to Bureau inmates.
• DEA Controlled Substance Registration Certificates must be submitted for all staff with prescriptive authority.
• Professional licenses must be submitted for each applicable staff member. License verification printouts are not accept-able substitutes, but may be submitted with original licenses to verify current expiration dates.
• A provisional license is not acceptable as a valid license to provide services. Interns, trainees or unlicensed clinicians must work directly (i.e., in the same room) with a licensed staff member when providing direct clinical services.
• Quoter must provide the State statutes that address the scope of practice for each license submitted.
• Any intended use of Telepsychiatry must be detailed and address the system’s ability to meet the Bureau’s electronic secur-ity requirements, as well as staffing procedures to ensure inmates are physically with a cleared staff member at all times.
PRICING
Prices proposed to the Government shall be as low as or lower than those charges to the supplier’s most favored customer for compar-able quantities under similar terms and conditions, in addition to any discounts offered for prompt payment.
DELIVERIES
In accordance with Federal Acquisition Regulation (FAR) 16.505, and the clause located in FAR 52.216-18, Ordering, states that any supplies or services to be furnished under this contract shall be ordered by issuance of task orders by a warranted Contracting Officer in the CTS Contracting Office.
BILLING INFORMATION
The Government will make payment on a monthly basis via Electronic Fund Transfer. Invoices with the contract number should be submitted on a monthly basis in duplicate to the Regional TDAT Coordinator indicated in Item 15 of the SF-1449. Contractors that are classified as a small business are required to indicate their small business size status on each invoice submitted for payment.

For three of the five years immediately prior to submission of an offer/bid/quote, or prior to performance under a contract or commit-ment, individuals or contractor employees providing services must have:

2. worked for the U.S. overseas in a Federal or military capacity; or

52.218-000 CONTINUING CONTRACT PERFORMANCE DURING A PANDEMIC INFLUENZA OR OTHER NATIONAL EMERGENCY (May 2008)
During a Pandemic or other emergency we understand that our contractor workforce will experience the same high levels of absentee-ism as our federal employees. Although the Excusable Delays and Termination for Default clauses used in Government contracts list epidemics and quarantine restrictions among the reasons to excuse delays in contract performance, we expect our contractors to make a reasonable effort to keep performance at an acceptable level during emergency periods.
The Office of Personnel Management (OPM) has provided guidance to federal managers and employees on the kinds of actions to be taken to ensure the continuity of operations during emergency periods. This guidance is also applicable to our contract workforce.
Contractors are expected to have reasonable policies in place for continuing work performance, particularly those performing mission critical services, during a pandemic influenza or other emergency situation.
The types of actions a federal contractor should reasonably take to help ensure performance are:
• Encourage employees to get inoculations or follow other preventive measures as advised by the public health service.
• Cross-train workers as backup for all positions performing critical services. This is particularly important for work such as guard services where telework is not an option.
• Implement telework to the greatest extent possible in the workgroup so systems are in place to support suc-cessful remote work in an emergency.
• Communicate expectations to all employees regarding their roles and responsibilities in relation to remote work

• Establish communication processes to notify employees of activation of this plan.
• Integrate pandemic health crisis response expectations into telework agreements.
• With the employee, assess requirements for working at home (supplies and equipment needed for an extended telework period).
