1. GENERAL: The Contractor shall provide laboratory courier services which include but are not limited to transporting laboratory specimens, reports, supplies, and other materials to and from designated locations identified in this Performance Work Statement (PWS) for the Phoenix Indian Medical Center (PIMC), Phoenix, AZ and Salt River Clinic, Scottsdale, Arizona at the locations listed in items 1.1., 1.2., and 1.3. below:

1.4 Resultant contract shall be non-personal in nature, as defined by FAR 37.101, and as such, shall be provided independent of Government supervision.
2. DESCRIPTION OF SERVICES: The contractor shall provide all supervision, labor, parts, materials, tools, supplies, vehicle, maintenance, and equipment necessary to provide Laboratory Courier Services as defined in this PWS and supporting documents at geographic locations as detailed in PWS Section 1.
2.1. The contractor shall obtain and maintain all required Federal, State, and Local licenses associated with providing Laboratory Courier Services. Performance shall be according to the PWS and supporting documents and shall also conform with:
2.1.1. Protected Health Information (PHI)
2.1.2. Health Insurance Portability and Accountability Act (HIPPA)
2.1.3. Indian Health Service (IHS) Standards
2.1.4. Standards of Procedures (SOP) – Proper Medical Transport Equipment
2.1.5. Current Valid Driver’s License
2.1.6. US Department of Transportation – Transport of Clinical Specimen Training Certificate (Annual Requirement)
2.1.7. Centers for Disease Control – Transport of Clinical Specimen Training Certificate (Annual Requirement)
2.1.8. Universal Precautions for Biohazard Material Handling Training Certificate (shall include proper procedures for spills/accidents). Annual Requirement.
2.2. Contractor shall be responsible for the timely pick-up/delivery (also referred to as a “stop”) of laboratory specimens, including but not limited to specimen categories: anatomic/surgical pathology, chemistry, cytology, blood bank, coagulation, cytogenetics/molecular genetics, hematology, immunology/serology, immunocytology and microbiology. Other items include
Phoenix Indian Medical Center reports, supplies and other materials (e.g., specimen containers, pre-packed specimens, etc.) to and from designated sites. PIMC Laboratory and Salt River Clinic shall be responsible for collecting the specimens from the patients and preparing them for pick up by the Contractor in accordance with the terms and conditions of this PWS and supporting documents.
2.3. The PIMC Laboratory/Pathology Secretary and/or Director of Pathology or his/her authorized designee are the only personnel authorized to submit paperwork for modification to add, delete, or change facilities and routine routes to meet patient or departmental needs. The paperwork shall be submitted to the Contracting Officer at the Phoenix Area Office (PAO) who in turn shall send the Contractor an approved, signed, and executed modification to the contract. NOTE: The authority to negotiate changes in the terms and conditions and/or the amounts cited in the contract are reserved to the Contracting Officer only. All changes shall be made in accordance with the Contract and Modifications. Additional route stops shall be paid at the same rate as set forth in this Contract.
2.4. Contractor shall establish and utilize a comprehensive Quality Control Plan (QCP) to assure
PIMC a consistently high level of performance throughout the term of the contract in providing laboratory courier services herein that meet or exceed Federal Department of Transportation (DOT); Hazmat Materials Regulations (HMR); Health Insurance Portability and Accountability Act (HIPAA) compliancy, and other State, Federal, and local laws, ordinances, regulations, rules, and directives and any other appropriate accreditation or licensing agency regarding transport of biologic specimens, prescriptions, transport containers, specimen handling procedures, labeling of transport containers. QCP shall be submitted with offer. QCP shall include, but is not limited to, the following:
2.4.1. Method of monitoring to ensure that contract requirements and the quality assurance as prescribed by State and Federal Regulations are met.
2.4.2. Method of keeping quality control records in accordance with State and Federal Regulations on transporting/handling of biohazardous materials, diagnostic specimens and spill cleanup.
2.4.3. Method of providing proper conditions for the transport of biologic specimens, transport containers, including proper preservation of samples, or other alterations of the sample which may affect sample testing and/or integrity.
2.4.4. Contractor shall have an ongoing system of quality assurance/ improvement and shall keep quality control records and records of all inspections conducted by Contractor. These records, which shall include, but are not limited to, any corrective action taken, the time a problem was first identified, a clear description of the problem and the time elapsed between identification and completed corrective action, shall be provided to the County upon request.

2.5. Drivers shall be trained in the transport/handling of biohazardous materials, diagnostic specimens/etiologic agents, biohazard spill cleanup, and HIPAA familiarization and compliance per these regulations.
2.6. PIMC responsibilities are as follows:
2.6.1. Shall provide the Contractor with specimens and other items that are appropriately labeled in accordance with PIMC Laboratory guidelines and directives for transport.
2.6.2. Laboratory specimens and other items shall be packed for pick up at facility and transported by the Contractor’s laboratory courier service.
2.6.2.1. PIMC shall be responsible for the preparation and packaging of specimens and other items for transport to designated sites.
2.6.2.2. Specimens shall be labeled appropriately and contained in locked totes.
2.6.3. The Government shall package and/or bag specimens in accordance with Occupational Safety & Health Administration (OSHA), State, Federal, Local laws, IHS Regulations and Rules to maintain specimen integrity for transport.
2.6.4. The Government shall provide specimen bags, specimen containers, or Styrofoam containers etc. on site, in preparation for subsequent packaging and transport by Contractor.
2.7. Contractor responsibilities are as follows:
2.7.1.Provide a Contractor’s Point of Contact (CPOC) or authorized designee who shall act as the central point of contact with PIMC. PIMC shall have access to the CPOC, or authorized designee, as detailed in PWS, Section 3.
2.7.2. Assign a sufficient number of experienced drivers to provide service to all specified Routine Routes. Contractor shall ensure that all specimen pick up drivers have the following:
2.7.2.1. Have a valid Arizona driver’s license (not suspended or revoked), and maintain automobile insurance in accordance with the terms and conditions of this PWS and supporting documents.
2.7.2.2. Trained in biohazardous materials handling, blood borne pathogens exposure and spill cleanup in compliance with United States Department of Transportation (DOT), The Centers for Disease Control and Prevention (CDC) transportation regulations, State and Federal Regulations and all applicable regulatory agencies.
2.7.2.3. Assure samples and other transported supplies/items are transported in accordance with the terms and conditions of the PWS and supporting documents.
2.7.2.4. Trained in Health Insurance Portability and Accountability Act of 1996
(HIPAA).
2.7.3. Ensure adequate staffing of drivers and availability of courier vehicles to provide Routine Route and Non-Routine backup coverage for illness, holidays, or other absences of assigned drivers, for example:

2.7.3.1. If a primary driver is unavailable.
2.7.3.2. If a vehicle is disabled or out of service.
2.7.3.3. To provide non-routine services (STAT & Super-STAT services, see PWS Section 2.12).
2.7.4. Contractor personnel shall wear uniforms with the name of company on the uniform. An identification badge with picture of the driver, name of driver, and company name is required to be worn at all times while performing contracted services. No T-Shirts, shorts, sandals, or open-toed shoes are permissible.
2.7.5. Contractor personnel shall comply with all facilities policies regarding smoking. The IHS facilities are non-smoking facilities.
2.7.6. Contractor shall ensure that its employees satisfy the health and safety standards set forth in the contract and all applicable State and Federal regulations, ordinances, and directives including initial and annual blood borne pathogen exposure training.
2.7.7. Contractor personnel performing work shall be fingerprinted, background checked, and cleared before work performance can begin.
2.7.8. Inappropriate behavior by Contractor personnel shall not be tolerated. Inappropriate behaviors include, but are not limited to, improper physical actions (touching, fondling), improper verbal statements (using derogatory comments, slurs, verbal abuse, etc.), and improper visual actions (leering, making sexual gestures). No weapons are allowed at any IHS facilities.
Contractor personnel shall not bring Non-Contractor employee visitors onto PIMC grounds and the Laboratory/Pathology Department. Contractor personnel may be subject to security screening at all IHS facilities.
2.8. At each designated facility, laboratory specimens shall be picked up and delivered to other facilities. The Contractor shall also pick up and deliver other materials requiring transport between designated facilities, both public and private; including hospitals and clinics, institutions, and businesses. Other facility locations are STAT/Super Stat runs/trips and are listed in table below:

2.9. Contractor shall provide training and continuing in-service training to all of its personnel, prior to providing services. This training is at the expense of the Contractor and shall not be billed to the Government.
2.9.1. Contractor’s staff providing services under this PWS and supporting documents shall be fully trained in their assigned tasks and in the safe transport/handling of specimens and equipment.
2.9.2. Contractor’s staff shall be trained in handling biohazardous materials, and biohazard spill cleanup per Federal DOT.
2.9.3. All contractor equipment shall be checked daily for safety. While handling biohazardous materials and/or biohazard spill cleanup, all employees shall wear safety and protective gear according to State and Federal Regulations and OSHA standards.
2.10. Contractor shall provide documentation of training program completion for all employees, including new employees, who provide services.
2.11. Contractor shall provide specimen handling procedures for all temperatures, communication methods, backup procedures, driver safety training procedures/documentation, and safety/biohazardous spill procedures with offer.
2.12. Contractor shall be responsible for providing laboratory courier services for IHS facilities on both a Routine and Non-Routine basis. Non-Routine laboratory courier services include “STAT” and “Super STAT”.
2.12.1. Contractor’s laboratory courier drivers providing services shall be trained and capable of being notified on short notice to pick-up/deliver laboratory specimens, etc. as part of Non- Routine services.
2.12.2. “STAT” services are requests requiring the transport of specimens or other materials from one (1) designated facility to another designated facility one (1) to four (4) hours after courier service is notified.
2.12.3. “Super STAT” services are requests requiring the transport of specimens or other materials from one (1) facility or Non-IHS facility within one (1) hour of the time the Contractor’s courier dispatch receives the request from PIMC.
2.13. Laboratory courier personnel/drivers shall not allow passengers in their vehicles during the hours where contractor personnel are performing PIMC services, other than Contractor personnel who have been trained in like-manner in the handling of biohazardous materials, nor bring non-contractor visitors into facilities. Transportation of PIMC staff in Contractor’s vehicles is not allowed. Contractor shall ensure that the vehicles used in the performance of services meet applicable State, Federal and local laws and regulations relating to safe vehicle operation.

2.14. Contractor shall assure integrity of specimens during transport by maintaining specimens at temperatures coinciding with PIMC facility’s specimen storage conditions at the time of pick-up or as directed by PIMC facility personnel. This includes maintaining specimen temperature storage conditions from facility pick-up site to vehicle and vehicle to delivery site. Required storage temperatures are:
2.14.1. Room temperature (20-25°C/68-77°F)
2.14.2. Refrigerator (2-8°C/36-46°F)
2.14.3. Frozen (< -15°C/< 5°F)
2.14.4. Body Temperature (35-39°C/95-102°F)
2.15. Transport of other materials, such as supplies/reagents, shall require separate containers other than those used for specimens.
2.16. In case of specimen breakage or leakage during transport, Contractor’s driver shall notify PIMC COR at the location or facility where specimens were picked up, in addition to complying with biohazard spill clean-up procedures and biohazard notification as required by State and federal regulations.
2.17. There shall be a PIMC inquiry into any loss of laboratory specimens, envelopes, supplies, documents, etc. in connection with their receipt, transport, or delivery by Contractor. If, after the inquiry, fault is found with the Contractor, the Contractor may be held liable for any damages resulting from the loss, including but not limited to replacement of the lost item(s) or subsequent patient damages from loss of specimen(s) or patient records and subsequent HIPAA violations.
2.18. Contractor shall ensure that the PIMC COR or his/her designee at each impacted facility be notified of any delay or disruption of services that may possibly exceed thirty (30) minutes beyond the expected time of Contractor’s driver arrival at the facility. Notification shall include the reason for the disruption and an estimated time of arrival for the completion of services. The reason for the delay shall also be recorded on the Daily Courier’s Manifest.
2.19. Courier may park free in designated parking spaces for visitors at the PIMC back Laboratory entrance. Free parking shall also be available for the courier at each pick-up and delivery facility.
3. CONTRACTOR’S POINT OF CONTACT: The contractor shall provide a Point of Contact (CPOC) who shall be available via telephone 24/7/365 (including weekends and holidays).
3.1. The CPOC shall be responsible for the performance of the work.
3.2. The CPOC shall have full authority to act for the contractor on all matters relating to daily operations.

3.3. The contractor shall designate the CPOC in writing to the Contracting Officer (CO) before the contract start date.
3.4. An alternate may be designated, but the contractor shall identify those times when the alternate will be the primary CPOC.
4. CONFLICT OF INTEREST: The contractor shall not employ any person who is an employee of the
United States Government if the employment of that person would create a conflict of interest.
5. ENGLISH LANGUAGE REQUIREMENT: Contractor’s personnel shall read, understand, speak, and write English fluently.
6. WORK SCHEDULE: The Contractor SHALL provide 24/7/365 (including weekends and holidays) response to laboratory courier service calls. Laboratory courier service shall include Routine and Non- Routine Routes to PIMC, Salt River Clinic, and other Non-Routine stops.
6.1. The scheduled (routine) pick-up of laboratory specimens shall be twice daily, to be transported from Salt River clinic to PIMC Monday thru Friday (see PWS Section 6.1.1. for Tuesday schedule). The first scheduled pick-up is between 11:30 a.m. – 12:00 p.m. The second scheduled pick-up is between 4:30 p.m. – 5:00 p.m.
6.1.1. The Contractor shall pick-up laboratory specimens three times daily to be transported from Salt River Clinic to PIMC on Tuesdays. The first scheduled pick-up is between 11:30 a.m.
– 12:00 p.m. The second scheduled pick-up is between 4:30 p.m. – 5:00 p.m. The third scheduled pick-up is between 6:30 p.m. – 7:00 p.m.
6.3. Specific routine route schedule and pick-up/delivery times are provided in PWS. Routes have been arranged to accommodate each designated facility’s hours of operation. The Contractor laboratory courier routes may extend from morning hours to evening hours. The Contractors are mandated to promptly contact PIMC Laboratory/Pathology Secretary and/or Contracting Officer Representative (COR) or his/her designee, if deliveries cannot be made or will be substantially delayed beyond thirty (30) minutes from the scheduled pick-up/delivery time. Hours/days of work may include Non-Routine services to and from facilities or alternate locations identified at the time services are requested by PIMC. Routine route pick-up/delivery times are determined by consideration of the distance proximity of the designated facility and the hours of that facility’s accessibility. Routine routes are designated to transport laboratory specimens, laboratory supplies, and other materials to and from the facilities in the most efficient and timely fashion possible. NOTE: Contractor requests for changes to routine routes or times shall be pre-approved by the COR or authorized designee, and PAO Acquisitions or authorized designee.

6.4. Contractor shall communicate with the contact person at each pick-up facility and the
Contracting Officer should weather conditions delay or cause cancellation of services. Due to the nature of this business it is necessary for the Contractor to furnish vehicles that operate safely in inclement weather conditions. Cancellation should only occur as a last resort or due to Acts of God.
7. CONTRACTOR FURNISHED SUPPLIES, EQUIPMENT, FACILITIES, AND PROPERTY:
7.1. VEHICLES: All vehicles used in the performance of services are required to be:
7.1.1. Enclosed automobiles, vans, or trucks
7.1.2. Air conditioned
7.1.3. Vehicle security devices shall include door locks and audible alarm
7.2. SPECIMEN TRANSPORT CONTAINERS: The Contractor shall supply all specimen transport containers required:
7.2.1. Specimen Transport container Specifications
7.2.1.1. Transport containers shall adhere to DOT, Arizona/OSHA and other State, Federal and Local Laws, Ordinances, Regulations, Rules, and Directives.
7.2.1.2. Contractor shall maintain a description of specimen transport containers (refrigerators, freezers, coolers, carriers, heating blocks, warm and cold packs, dry ice, etc.) utilized within vehicles to contain specimens and other laboratory supplies during transit and containers used for specimen transport to and from each facility to vehicle
(e.g., insulated shoulder carrier bags). Proper labeling and construction of such specimen transport containers are the responsibility of the Contractor and shall adhere to DOT, Arizona/OSHA and other State, Federal and local laws, ordinances, regulations, rules and directives. Contractor shall submit to PIMC, upon request, the description of specimen transportation being utilized. PIMC has the right to approve or disapprove Contractor’s use of specimen transport containers.
7.3. NON-SPECIMEN TRANSPORT CONTAINERS: The Contractor shall supply all non-specimen transport containers required. Tote/bin provided for the supplies/materials that are to be transported shall be a maximum size of 30” (length) X 20” (depth) X 18” (height) for routine trips/runs. For the STAT/Super STAT runs Maximum size of 19” (length) X 9” (depth) X 9” (height).
7.4. All vehicles used to perform services shall be registered, licensed, and meet the minimum vehicle requirements as mandated by the Department of Transportation. The Government reserves the right to inspect the vehicles and investigate the responsibility of the Contractor to perform the work outlined in this contract and reject any offer should contractor fail to comply with these requirements.

7.5. Vehicle size: Any vehicle that can transport a tote with a maximum size of 30” (length) X 20”
(depth) X 18” (height).
7.6. All vehicles shall be in good mechanical condition, with body in good state of repair, clean and appearance throughout, to assure high-quality transportation. All vehicles shall be equipped with working heaters and air conditioners.
7.7. All vehicles shall contain equipment capable of two-way verbal communication between persons on the transport vehicle, the contractor’s dispatch location, and destination location.
7.8. All vehicles shall be covered by necessary insurance, licenses, and/or permits to perform all required activities.
7.9. All Contractor personnel shall obey all motor vehicle laws.
8. HEALTH REQUIREMENTS: Contractor personnel shall present a clean and neat appearance at all times.
9. STAFFING AND PROFESSIONAL QUALIFICATIONS CRITERIA: The Contractor shall possess all necessary licenses and/or permits required to perform all work as defined in PWS Section 2 and Supporting Documents.
9.1. All drivers shall be of legal age, possess a valid driver license, and maintain a neat, clean, and professional appearance.
9.2. Drivers shall be able to speak, understand and write English fluently.
9.3. Drivers shall possess identification showing Contractor name and job title.
9.4. Contractor is responsible for the actions and conduct of its employees and shall assure that they are consistently compliant with all PIMC building and parking regulations, policies, and guidelines while conducting business on Government property and for PIMC.
9.5. Drivers shall not make any personal stops in performance of duties nor shall any unauthorized passenger accompany the driver while performing services.
9.6. Drivers shall observe the smoking policy of the PIMC and only smoke in designated areas.
9.7. Drivers transporting laboratory specimens shall be trained in Universal Precautions for handling of biohazard materials and include use of proper procedures in case of spills or accidents. This training shall also include mandatory Hazardous Materials training. Written statements documenting Hazardous Materials training and training that meets the laws and regulations of the US Department of Transportation and the Centers for Disease Control governing the transport of clinical specimens shall be provided to the COR annually.

10. REQUIRED INSURANCE: Contractor shall meet all Federal, State, and City codes regarding operation of this type of business and furnish the required insurance certificates required to work on a Government installation.
10.1. The Contractor shall indemnify and hold the Government harmless for any and all losses, damages, or liability on account of personal injury, death, or property damage, or claims for personal injury, death, or property damage of any nature whatsoever and by whomsoever made, arising out of the activities of the Contractor, his employees, subcontractors, or agents. For the purpose of fulfilling his obligations under this paragraph, the Contractor shall procure and maintain during the term of this contract, and any extension thereof, liability insurance in form satisfactory to the Contracting Officer. The named insured parties under the policy shall be the Contractor, the United States of America, their Agents, and their employees.
10.2. The amount of the insurance shall be not less than $100,000 each person, $300,000 each occurrence, and $100,000 property damage. The Contractor shall furnish to the Contracting Officer, prior to commencement of performance, evidence of insurance coverage described herein. Insurance shall be in place throughout the entire period of performance.
11. QUALITY CONTROL PROGRAM (QCP): The Contractor shall develop and maintain an effective
QCP to ensure services are performed in accordance with this PWS. The Contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. The Contractor’s QCP is the means by which it assures that work is in compliance with requirements as defined in the PWS and supporting documents. The QCP is part of the evaluation of offers.

12. PERFORMANCE REQUIREMENTS SUMMARY (PRS):
12.1. Performance Requirements Summary:
12.1.1. The Contractor service requirements are summarized into performance objectives that relate directly to mission essential items. The performance threshold briefly describes the minimum acceptable levels of service required for each requirement. These thresholds are critical to mission success.

The Contractor shall provide timely transport of Routine Laboratory specimens

The Contractor shall provide timely transport of STAT Laboratory specimens
Contractor shall transport STAT specimens within 2 hours of Specimen Transport Log

The Contractor shall maintain specimen integrity.
The specimen integrity shall be maintained at optimal temperatures required.
Specimen integrity shall be maintained 100% of the time.
Based on Provider Notification of: Specimen and Patient Non- Compliance, and Changes to Verified Lab Data Form
The Contractor shall provide timely notification of any delivery delays
No delay or disruption of services that exceeds thirty (30) minutes beyond the required delivery time.
Less than 3 unfavorable incidences per quarter
