About this file

This document is a Request for Quote (RFQ) issued by the Illinois Department of Agriculture. The agency is seeking to purchase two (2) Scantron Insight 4ES OMR scanners to add to their existing six (6) Scantron scanners, and is requesting a five (5) year maintenance agreement on all eight (8) scanners. The scanners are used for grading certification tests for the pesticide applicator certification and licensing program. The RFQ outlines detailed specifications for the maintenance services, including repair or replacement of parts, preventative maintenance, and response times for service calls. The vendor must agree to provide loaner equipment upon equipment failure and cover all transportation and travel costs.

The RFQ provides pricing instructions, requiring vendors to quote a price per machine for the two new scanners and a total price for the five-year maintenance agreement on all eight scanners. Vendors must agree that quoted prices will be valid for 90 days and include all shipping and handling charges, with no price increases accepted. The agency reserves the right to reject any or all offers, and the winning vendor will be selected based on the lowest total bid.
