About this file
This document is a Request for Quotation (RFQ) issued by the Ohio Department of Natural Resources (DNR) for East Fork State Park campground in Batavia, Ohio. The project involves the purchase, demolition, and installation of 9 new man doors and frames at three shower houses, with specific requirements including 6 doors labeled "MEN" and "WOMEN" with etched frosted glass, and 3 additional doors with plain frosted glass panels. Bidders are required to complete a mandatory site visit by contacting DeWayne Terwilleger at (740) 624-6348 to evaluate and measure for proper fit and finish. The project requires removal of existing exterior doors, installation of new commercial-grade hollow metal doors with transom frames, and replacement of all associated hardware including hinges, gasketing, door sweeps, thresholds, and tamperproof plates. The contract will be awarded to the lowest responsive and responsible bidder, with the project to be completed as soon as possible and no later than June 30, 2025.
Pricing and invoicing will follow Ohio state procurement guidelines, with invoices to be submitted directly to Ohio Shared Services via email, fax, or mail. The invoice must include specific details such as the Purchase Order number, supplier information, service address, itemized charges, and completed work date. No state sales tax will be applied, and the total contract value will be determined by multiplying unit costs by estimated usage. The procurement is funded through the DNR's budget, with a clear expectation that any services after June 30, 2025, will require a new Purchase Order. Bidders must quote on all items to be considered responsive, and the final award will be based on the lowest lot total calculation.
