B.3 PERFORMANCE WORK STATEMENT

MIAMI HOME OXYGEN SERVICE

1. SCOPE: This Performance Work Statement (PWS) defines the effort required for the setup, installation, and management of Home Oxygen and Ventilation, in accordance with Joint Commission (JC) standards for Homecare, to Veteran beneficiaries serviced by Miami VA Healthcare System (MVAHCS). This PWS includes the associated program management, human engineering, and logistic support planning requirements. The use of the terms, “beneficiary”, “Veterans”, “Patients”, and “Patient” are used interchangeably and refer to the recipient of required supplies, equipment, and services required under the contract.

1.1. BACKGROUND. The acquisition of Home Oxygen is being initiated to provide Home Oxygen to Veteran beneficiaries serviced by Miami VA Healthcare System. MVAHCS does not have the capacity to provide these supplies, so a contract has been sought for the provision of home oxygen and all associated products and services. Per VHA Directive 1173.13(1) Home Oxygen Program 5.0., the prescribing provider is responsible for completing the annual prescription for home oxygen for veteran patients. The Miami VA Healthcare System currently supplies an estimated 197 oxygen using patients in the catchment area of Dade, Broward and Monroe counties. The consult includes the patient’s oxygen prescription, oxygen prescription expiration date, equipment, supplies, and services to be provided including the date and place of delivery. All services shall be provided either at MVAHCS, the veteran’s residence, or in nursing home where the veteran resides. All efforts in support of this requirement are carried out in accordance with Joint Commission standards.

1.2. OBJECTIVES. The Government intends to award a single award fixed price Indefinite Delivery Contract as a result of this solicitation. In accordance with the prescription received from the MVAHCS, the contractor shall provide the beneficiary with home oxygen equipment and supplies. In addition, the contractor shall deliver, set up, and install all equipment and supplies at the veteran patient’s residence.

This full-service contract includes establishing operational parameters of equipment for VA prescription, ensuring purity of O2, veteran training on system operations, review of safety checks, and discussion of home safety issues. The operational parameters shall also include scheduled maintenance visits, emergencies, assessment requirements, and removal of equipment upon discontinuation notification.

Prices quoted shall be all inclusive of deliveries and transportation within the MVAHCS catchment areas, and equipment, safety inspections, repairs, maintenance, and in-service training for beneficiaries to include patient education.

1.2.1 The contractor shall provide all personnel, equipment, tools, materials, supervision, other items, and non-personnel services necessary to perform Home Oxygen Services for beneficiaries of the Miami VA Healthcare System (MVAHCS) catchment areas in Miami, Florida. The Contracting Officer Representative (COR) or VA designee shall provide the contractor with the prescription orders for veteran patients. The contractor shall accomplish the delivery, set-up, installation of equipment, assessments, patient education and scheduled maintenance. The contractor shall install and maintain all VA owned equipment including providing all related supplies. All Government Furnished Property (GFP) will be labeled as VA Owned by the contractor.

1.2.2 The Contracting Officer Representative (COR) or designee (home oxygen clinical staff) will provide the Contractor with notification of individual patient equipment requirements via fax or encrypted email or other electronic means (telephonic for emergencies). The VA home oxygen clinics in the three counties will be notified by the contractor via fax or encrypted email to confirm receipt of order(s) immediately. The contractor shall contact the home oxygen clinic or COR for assistance in clarifying orders.

1.3. PERIOD OF PERFORMANCE. The period of performance shall be for one (1) base year of 12-months and four (4) 12-month option years. The period of performance reads as follows:

• Base Year

• Option Year One

• Option Year Two

• Option Year Three

• Option Year Four

1.3.1 The government reserves the right to extend the term of this contract at the prices set forth in Section B in accordance with the terms and conditions contained in clause 52.217-9 entitled, “Option to Extend the Term of the Contact.”

1.4 GEOGRAPHIC AREA OF COVERAGE. The Contractor shall provide Home Oxygen within a defined area of responsibility/jurisdiction to the patients’ residence, or other location as indicated on the order for Home Oxygen.
