Services will be provided on-site at Phoenix Indian Medical Center, 4212 North 16th Street, Phoenix, Arizona. Two labor categories require pricing: Diagnostic Radiologic Technologist – Mammography and Diagnostic Radiologic Technologist – Computed Tomography. The BPA will have no base period or option periods; services will be ordered through individual BPA Calls as requirements arise. Quotation due date is September 1, 2026, at 2:00 PM Arizona Time, submitted electronically to dekovan.cook@ihs.gov. Questions must be submitted by August 28, 2026, at 4:00 PM Arizona Time. All quoted hourly rates must be fully burdened and include wages, fringe benefits, recruiting, credentialing, onboarding, management, quality control, certifications, training, insurance, travel, lodging, meals, transportation, airfare, rental vehicles, taxes, overhead, and profit. No separate reimbursement will be provided for costs included in the hourly rates. The Government will not provide housing, nor will it pay overtime, holiday, weekend, or shift differential premiums separately; all authorized hours will be compensated at the firm-fixed hourly rate. Contractor personnel must meet all qualification requirements including ARRT certification, modality-specific certifications, BLS, MQSA (for mammography), and security/background clearances before beginning services. Evaluation will be based on comparative evaluation of Technical Capability and Relevant Experience/Past Performance (combined as significantly more important than price) and Price. The Government intends to award to the responsible, eligible ISBEE quoter providing the most advantageous solution.

The Department of Health and Human Services (HHS), Indian Health Service (IHS), Phoenix Area Indian Health

Service, is issuing this Request for Quotations (RFQ) for nonpersonal Diagnostic Radiologic Technologist (DRT) services in Mammography and Computed Tomography in support of Phoenix Indian Medical Center (PIMC).

This acquisition is for commercial services and is being conducted using the simplified procedures of HHS

Revolutionary FAR Overhaul (RFO) FAR Part 12, Acquisition of Commercial Products and Commercial Services.

The Government intends to establish one Single-Award Blanket Purchase Agreement (BPA) pursuant to HHS

RFO FAR 12.201-1(e)(3).

The BPA will establish firm-fixed, fully burdened hourly rates, terms, conditions, and ordering procedures for recurring DRT services during a five (5) year ordering period. The BPA will not contain a base period or option periods. Services will be ordered through individual BPA Calls as requirements arise.

This acquisition is restricted to eligible Indian Small Business Economic Enterprises (ISBEEs) pursuant to the Buy

Indian Act and applicable HHS/IHS requirements. The Government intends to establish the BPA with the responsible, eligible ISBEE Quoter whose quotation is determined to provide the most advantageous solution to the Government in accordance with Section D of this solicitation.

A.2 Solicitation Clarifications - Read Before Submitting Questions

The following information is provided to address common questions concerning healthcare staffing solicitations.

Quoters shall prepare quotations based on this solicitation and its attachments. Questions requesting information already addressed below or elsewhere in the solicitation may be answered by reference to the applicable solicitation section.

Incumbent/Current Contract Information and Service Contract Labor Standards. Quoters shall develop their staffing approach and pricing based on the requirements, estimated hours, applicable Department of Labor Wage

Determination, and evaluation criteria contained in this solicitation and its attachments. The Government is not providing incumbent contractor pricing, labor rates, staffing levels, or other proprietary or procurement-sensitive incumbent information for quotation-development purposes.

Government-Furnished Housing, Travel, Lodging, Meals, and Transportation. Government-furnished or on-campus housing is not provided or guaranteed. Quoted hourly rates shall be fully burdened and include all travel, lodging, meals, local transportation, airfare, rental vehicle, and other costs necessary to perform the requirement.

No separate reimbursement will be made unless expressly identified in the resulting BPA or an individual BPA

Call.

Government-Furnished Medical Equipment and Systems. PIMC will provide the medical imaging equipment, medical supplies, workspace, and Government information-system access necessary to perform on-site services as identified in the PWS. Contractor personnel are responsible for personal items and other Contractor-furnished resources identified in the PWS.

Orientation and Mandatory Training. Contractor personnel shall complete applicable PIMC orientation and mandatory facility-specific training required by the PWS. Government-required orientation or facility-specific training is compensable only when authorized under the applicable BPA Call and performed during authorized service hours. Contractor costs associated with maintaining professional certifications, continuing education, and other Contractor personnel qualifications are included in the fully burdened hourly rate and are not separately reimbursable.

Personnel Qualification and Clearance Requirements. Contractor personnel shall satisfy all applicable qualification, certification, employee-health, security, background, orientation, and facility-access requirements identified in the PWS before beginning services. The Contractor is responsible for timely submission of required documentation. Government review or verification of personnel qualifications does not constitute Government hiring, selection, supervision, or employment of Contractor personnel.

Single-Award BPA. The Government intends to establish one Single-Award BPA. This is not a multiple-award

BPA.

Ordering Period. The BPA will have one five (5) year ordering period. There are no base-year or option-year periods.

Estimated Hours Are Not FTEs. Estimated hours are provided for acquisition planning, quotation evaluation, and BPA administration. They do not represent full-time equivalents (FTEs), guaranteed staffing positions, guaranteed schedules, or a required number of technologists. The Contractor is responsible for determining the number of qualified personnel necessary to satisfy required coverage.

Shift Distribution. Anticipated shift windows are identified in Attachment A, PWS. Actual coverage is based on patient-care needs, facility operations, staffing levels, and available funding. Required coverage will be identified under the applicable BPA Call and related scheduling requirements.

On-Call/Call-Back. No on-call or call-back services are currently required. If the Government later requires services outside the scope stated in the PWS, they will not be ordered without appropriate contractual authority.

Overtime/Premium Rates. The Government is not establishing separate overtime, holiday, night, weekend, or other premium labor categories or rates. All authorized hours performed will be compensated at the applicable firm-fixed hourly rate established for the labor category. The Contractor remains responsible for compliance with applicable labor laws and for compensation of its employees.

Personnel Selection. Representative resumes are requested to demonstrate recruiting capability and understanding of PWS qualifications. The Government is not hiring individual Contractor employees. The

Contractor remains responsible for recruiting, assigning, supervising, managing, and replacing its personnel, subject to PWS qualification, security, health, and access requirements.

Arizona License. A separate Arizona radiologic technologist license is not required for this requirement.

Contractor personnel shall satisfy the ARRT, modality-specific, BLS, MQSA, and other qualification requirements identified in Attachment A, as applicable.

Verification of Hours. The Government is not providing or requiring a separate Government timesheet attachment. The Contractor shall maintain and submit documentation sufficient for the Government to verify actual hours performed for payment purposes.

No Guaranteed Minimum. Establishment of the BPA does not guarantee issuance of BPA Calls, a minimum number of hours, a minimum quantity of services, or a minimum dollar value. Funds are obligated only through authorized BPA Calls.

Radiologic Technologist Services - Mammography and Computed Tomography. The PWS includes the Quality

Assurance Surveillance Plan (QASP).

SECTION B - BPA PRICING, ORDERING, AND PAYMENT

B.1 BPA Pricing

Quoters shall complete Attachment B - BPA Pricing Schedule. The BPA will establish firm-fixed, fully burdened hourly rates for the following labor categories:

• Diagnostic Radiologic Technologist - Mammography

• Diagnostic Radiologic Technologist - Computed Tomography

Quoted hourly rates shall include all direct and indirect costs necessary to perform the requirement, including wages, fringe benefits, recruiting, credentialing, onboarding, Contractor management and supervision, Contractor quality control, required certifications and training, insurance, travel, lodging, meals, transportation, airfare, rental vehicle, taxes, overhead, general and administrative expenses, profit, and other Contractor costs necessary to perform the requirement.

No separate reimbursement will be made for costs included in the established hourly rates unless expressly authorized in the resulting BPA or applicable BPA Call.

Unless expressly identified otherwise in the solicitation, resulting BPA, or applicable BPA Call, the Government will not separately reimburse airfare, lodging, meals and incidental expenses, rental vehicles, mileage, fuel, parking, credentialing costs, licensing/certification costs, employee health requirements, onboarding costs, or other Contractor expenses. Quoters shall account for such costs in their fully burdened hourly rates.

B.2 Estimated Hours

Estimated hours in Attachment B are provided solely for acquisition planning, quotation evaluation, and BPA administration. Estimated hours do not constitute guaranteed hours, guaranteed staffing positions, full-time equivalents, minimum purchases, or guaranteed revenue. The Contractor is responsible for determining the number of qualified personnel necessary to satisfy required coverage.

B.3 BPA Calls

Services shall be ordered only through written BPA Calls issued by the Contracting Officer or another

Government official expressly authorized to place Calls. Each BPA Call will identify, as applicable, the labor category, place of performance, required coverage, estimated hours, period of performance, applicable hourly rate, total Call value or not-to-exceed amount, funding, COR, and any Call-specific requirements within the scope of the BPA and PWS.

Requirements contained in individual BPA Calls shall remain within the scope of the BPA and PWS. The COR is not authorized to issue BPA Calls, obligate funds, modify the BPA or a BPA Call, or otherwise change the terms and conditions of the BPA unless separately delegated such authority in writing.

B.4 Hours in Excess of Scheduled Coverage

Services beyond scheduled coverage shall be performed only when authorized in advance in accordance with the BPA or applicable BPA Call. Unauthorized hours will not be reimbursed.

The Government is not establishing separate overtime, holiday, night, weekend, callback, shift differential, standby, on-call, or other premium labor categories or rates. All authorized hours performed, including services performed on weekends, Federal holidays, nights, or in excess of forty

(40) hours in a workweek, will be compensated at the applicable awarded firm-fixed, fully burdened hourly rate for the labor category.

The Contractor is solely responsible for complying with applicable Federal, State, and local requirements governing compensation of its employees. Any overtime compensation, holiday premium pay, shift differential, or other employee compensation required by law or by the Contractor's employment practices is the responsibility of the Contractor and shall not result in additional cost to the

Government.

B.5 Verification, Invoicing, and Payment

The Contractor shall maintain and submit documentation sufficient for the Government to verify actual services and hours performed. Documentation shall identify, at a minimum, the Contractor personnel performing services, date of service, applicable labor category, hours performed, and applicable BPA Call.

The Contractor shall submit invoices monthly unless otherwise specified in the applicable BPA Call. Each invoice shall identify the BPA Number, BPA Call Number, invoice number, billing period, labor category, dates of service, hours performed, applicable hourly rate, total amount billed, and other information required by the applicable payment instructions.

Payment will be made only for services authorized, actually performed, properly documented, verified, and accepted by the Government. Government verification of hours does not constitute supervision of Contractor personnel.

B.6 Service Contract Labor Standards

This acquisition is subject to the Service Contract Labor Standards statute and applicable HHS RFO FAR Part 22 requirements to the extent applicable to employees performing the requirement. The applicable Department of

Labor Wage Determination is incorporated as Attachment F. The Contractor is responsible for compliance with applicable wage, fringe benefit, and labor requirements, including any applicable conformance requirements for classifications not listed in the Wage Determination.

SECTION C - INSTRUCTIONS TO QUOTERS

C.1 General

Quoters shall submit complete quotations electronically in accordance with Section E. Quotations shall be clear, concise, and contain sufficient information for the Government to evaluate the quotation without assumptions regarding information omitted by the Quoter.

The Government may communicate with Quoters, request clarifications, or request revised quotations when determined appropriate by the Contracting Officer. However, the Government is not obligated to seek clarification or permit a Quoter to cure a material omission.

The Government will not assume or infer information omitted from a quotation. Information contained in one quotation volume will not be presumed to satisfy a requirement for another volume unless the Quoter clearly identifies the applicable cross-reference.

A quotation that fails to provide a required volume, required technical information, required pricing, or required

ISBEE documentation as specified below shall not be considered for BPA establishment.

Quoters shall provide only the information requested by this solicitation. General marketing materials, capability statements, brochures, and other information that does not directly address an evaluation factor are not required and may not be evaluated.

C.2 Quotation Organization and Mandatory Contents

Quotations shall contain all four volumes listed below. Each volume is mandatory.

• Volume I - Technical Capability

• Volume II - Relevant Experience and Past Performance

• Volume III - Price

• Volume IV - Buy Indian/ISBEE Documentation

Failure to submit any required volume shall result in the quotation not being considered for BPA establishment.

C.3 Volume I - Technical Capability

Volume I shall address each subsection below. Failure to address any required Technical Capability subsection shall result in the quotation not being considered for BPA establishment.

C.3.1 Recruitment and Staffing Capability

Describe the Quoter's demonstrated ability and approach to recruit qualified Mammography and CT technologists meeting the PWS requirements. At a minimum, address recruiting methods, recruiting pipeline, availability of qualified ARRT-certified personnel, ability to respond to recurring staffing requirements and changes in

Government-approved scheduled coverage.

C.3.2 Staffing Continuity and Replacement

Describe the Quoter's approach to retaining qualified personnel, maintaining accepted scheduled coverage, managing planned and unplanned absences, providing qualified replacement personnel, and minimizing disruption to PIMC operations.

C.3.3 Qualification Verification and Onboarding

Describe the Quoter's process for verifying and maintaining required ARRT certifications, Mammography/MQSA qualifications as applicable, CT qualifications, BLS, employee-health requirements, security/background requirements, facility onboarding, and expiration dates for required qualifications.

C.3.4 Contract and Personnel Management

Describe the Quoter's approach to Contractor personnel supervision, scheduling, quality control, performance monitoring, corrective action, communication with the Government, and administration of recurring BPA Calls.

The approach shall demonstrate that the Quoter, not the Government, will manage and supervise Contractor personnel.

C.3.5 Representative Personnel

Provide representative resumes for up to three (3) Mammography Technologists and up to three (3) CT

Technologists. Resumes shall demonstrate the Quoter's ability to recruit personnel meeting the PWS qualifications. Representative resumes do not constitute Government-selected key personnel or a commitment that the individuals shown must perform under the BPA.

The Government does not require the representative individuals submitted with the quotation to be available for assignment at BPA establishment.

C.4 Volume II - Relevant Experience and Past Performance

Provide up to three (3) recent and relevant contracts, BPAs, orders, or comparable commercial engagements performed within the past five (5) years. For each reference, provide the customer, contract/order number if applicable, period of performance, approximate dollar value, description of services, labor categories provided, customer point of contact, and explanation of relevance to this requirement.

Relevant experience may include Mammography staffing, CT staffing, radiologic technologist staffing, hospital diagnostic imaging, Federal healthcare, IHS or Tribal healthcare, or comparable healthcare staffing services. The

Government may consider information submitted by the Quoter and information independently available to the

Government.

Quoters are responsible for providing accurate and current customer point-of-contact information. The

Government is not obligated to locate alternate points of contact when information submitted by the Quoter is inaccurate or unavailable.

If the Quoter has no relevant past performance, the Quoter shall affirmatively state that it has no relevant past performance. A Quoter without relevant past performance will not be evaluated favorably or unfavorably solely on that basis. Failure to submit the required Volume II information or an affirmative statement of no relevant past performance shall result in the quotation not being considered for BPA establishment.

C.5 Volume III - Price

Quoters shall complete and submit Attachment B - BPA Pricing Schedule. All required labor categories and pricing periods in Attachment B shall be priced. Rates shall be firm-fixed and fully burdened.

The Government will calculate a total evaluated price using the estimated quantities identified in Attachment B.

The total evaluated price is used solely for evaluation and does not constitute a guaranteed BPA value or

Government commitment to order the estimated quantities.

Unit prices shall be entered for every required labor category and pricing period. Entries such as “N/A,” “TBD,” “To

Be Negotiated,” or similar qualifications will not satisfy a required pricing entry. Conditional pricing or pricing contingent upon separate reimbursement of costs identified by this solicitation as included in the fully burdened hourly rate shall not be accepted.

A quotation that fails to provide a price for any required labor category or full ordering period shall not be considered for BPA establishment.

C.6 Volume IV - Buy Indian/ISBEE Documentation

Quoters shall complete and submit Attachment D - Indian Economic Enterprise/ISBEE Representation and

Certification and all required supporting documentation. The Government will determine eligibility in accordance with applicable Buy Indian Act and HHS/IHS requirements.

A Quoter that fails to submit the required ISBEE representation/certification and supporting documentation, or that is determined ineligible for award as an ISBEE, shall not be considered for BPA establishment.

C.7 Assumptions, Exceptions, and Deviations

Quoters shall clearly identify any assumptions, exceptions, deviations, or conditions applicable to their quotation.

Any assumption, exception, deviation, or condition that materially alters a solicitation requirement, limits the

Quoter's obligation to perform the PWS, conditions pricing, or otherwise takes exception to a material solicitation requirement shall result in the quotation not being considered for BPA establishment.

Silence will be interpreted as the Quoter's agreement to perform in accordance with the solicitation and its attachments.

SECTION D - EVALUATION AND BPA ESTABLISHMENT

D.1 Comparative Evaluation

The Government will evaluate quotations using comparative evaluation procedures under HHS RFO FAR Part 12.

Quotations will be compared against one another to determine which quotation provides the most advantageous solution to the Government. Numerical scoring, adjectival ratings, and establishment of a competitive range are not required.

The Contracting Officer may communicate with Quoters and may request clarification or revised quotations when determined appropriate. Such communications do not obligate the Government to permit correction of a material omission or to conduct communications with every Quoter.

D.2 Evaluation Factors

The Government will evaluate the following factors:

• Factor 1 - Technical Capability

• Factor 2 - Relevant Experience and Past Performance

• Factor 3 - Price

Technical Capability and Relevant Experience/Past Performance, when combined, are significantly more important than Price. As quotations become more technically comparable, Price may become increasingly important.

D.3 Technical Capability

The Government will comparatively evaluate the Quoter's demonstrated ability to recruit qualified Mammography and CT personnel, maintain staffing continuity, retain qualified personnel, provide replacement personnel, verify and maintain qualifications, complete onboarding requirements, supervise and manage Contractor personnel, maintain quality control, and successfully administer recurring staffing requirements.

Greater consideration may be given to quotations demonstrating established recruiting pipelines for qualified

Mammography and CT technologists, demonstrated ability to maintain accepted scheduled coverage, and successful management of comparable healthcare staffing requirements.

D.4 Relevant Experience and Past Performance

The Government will comparatively evaluate the recency, relevance, scope, complexity, and quality of the

Quoter's prior performance. The Government may consider information provided by the Quoter and information obtained independently. A Quoter without relevant past performance will not be evaluated favorably or unfavorably solely on that basis.

D.5 Price

The Government will evaluate quoted hourly rates and total evaluated price for fairness and reasonableness.

Price analysis may include comparison among quotations, comparison with the Independent Government Cost

Estimate, comparison with historical prices, comparison with available commercial market information, and other price-analysis techniques considered appropriate.

D.6 Basis for BPA Establishment

The Government intends to establish one Single-Award BPA with the responsible, eligible ISBEE Quoter whose quotation is determined to provide the most advantageous solution after comparative consideration of Technical

Capability, Relevant Experience and Past Performance, and Price. The Government is not required to select the lowest-priced quotation.

The Government may incorporate portions of the successful Quoter's quotation into the resulting BPA.

Accordingly, statements and commitments made in the quotation may become binding requirements of the BPA.

SECTION E - QUOTATION SUBMISSION

E.1 Due Date and Submission

Time: 2PM Arizona Time

Submission Email: dekovan.cook@ihs.gov

E.2 Questions

Questions concerning this solicitation shall be submitted in writing no later than 4PM Arizona Time on August 28, 2026. All questions shall be submitted electronically to dekovan.cook@ihs.gov.

Before submitting a question, Quoters shall review the entire solicitation and all attachments, including Section

A.2. Questions shall identify the specific solicitation section, paragraph, or attachment requiring clarification.

The Government may respond to questions already addressed by the solicitation by identifying the applicable solicitation provision rather than restating the requirement. The Government may consolidate or paraphrase substantially similar questions.

Responses that materially affect the solicitation will be provided to all prospective Quoters through an amendment or other Government notice.

E.3 Quotation Validity
