About this file

This is an Addendum No. 1 to a Request for Quote (RFQ) issued by DeKalb County Public Health (DKPH) in Georgia for graphic design services to support the REACH LEAD DeKalb initiative focused on physical activity, nutrition, and healthy family weight. The RFQ originally required submissions by November 13, 2025, at 2:30 p.m. EST, but this addendum extends the due date to Tuesday, November 18, 2025, at 2:30 p.m. EST. All bids must be submitted via email to DPH-DCBOHpurchasing@dph.ga.gov with the subject line "26RFQ02 Graphic Design Services." The scope of work includes designing a Community Health Improvement Plan (CHIP) document layout (up to 70 pages), campaign look and feel materials using existing DKPH branding guidelines, six distinct print and online advertisements (static), twelve social media advertisements (primarily static with potential for animation), a direct mailer, and flyers. Some deliverables require Spanish-language versions. The contract period will begin upon execution and end on June 30, 2026, with four renewal options. All deliverables must be completed remotely in digital format and submitted in print-ready Adobe files, with DKPH retaining ownership of all artwork files. DKPH will handle printing, distribution, media placement, and mailing fulfillment internally.

The budget for this contract is undetermined. This project is supported by federal funds with a strict prohibition on subcontracting; the awarded vendor must perform all work directly. Only vendors located within the Metro Atlanta area are eligible to submit, excluding firms based outside this geographic region regardless of business registration status in DeKalb County. DKPH will provide finalized content, charts, data visualizations, branding guidelines, messaging frameworks, and visual assets to the contractor. The addendum addresses 36 questions and answers clarifying scope requirements, including that pricing may be provided in ranges based on CHIP page counts, optional pricing for expanded services is acceptable, and there is no incumbent vendor on this project. Questions regarding the RFQ were due by November 5, 2025, at 2:00 p.m. EST. All prospective bidders are responsible for monitoring the Georgia Procurement Registry and DKPH website for any additional addenda or communications related to the solicitation.
