About this file

This Request for Quotes (RFQ) #2023-01 is issued by CareOregon, Inc., a Medicaid managed care organization in Oregon, seeking consultant services to develop a comprehensive feasibility study for establishing a nonprofit, non-emergent ambulance service. The study will assess the viability of creating a new nonprofit ambulance company to provide basic life support, advanced life support, specialty care/critical care, stretcher-only, secure, and bariatric ambulance services dedicated to Medicaid members through the existing "Ride to Care" program, primarily serving the three-county Portland metro area (Multnomah, Clackamas, and Washington counties). The consultant must analyze business model considerations, regulatory requirements at local, state, and national levels, staffing feasibility for qualified EMTs, competitive impacts, financial requirements including startup costs and ongoing expenses, funding methods, sustainable business models, and potential barriers. Quotes were due by March 15, 2023, at 2:00 PM Pacific Time, submitted via email to Procurement@careoregon.org with "2023-01" in the subject line. The project is expected to last no longer than three months, with a deadline for clarifying questions set for March 8, 2023, at 5:00 PM PT.

The anticipated budget for this feasibility study is $70,000, with quotes to be submitted as a fixed fee including any travel and incidental expenses. Quoters must demonstrate experience in healthcare, transportation, and Medicaid; provide detailed workplans with project approach, milestones, and timelines; and submit at least three client references for similar projects. Required qualifications include company experience, staff qualifications, diversity and inclusion practices, financial stability documentation, and social media/litigation disclosures. All quotes are binding for 60 days from the due date and will be evaluated based on firm experience, staff experience, price/fees, references, and project completion proposals. The selected quoter must comply with specific insurance requirements including Commercial General Liability ($1,000,000 per occurrence/$2,000,000 annual aggregate), Professional Liability ($1,000,000 per occurrence/$2,000,000 annual aggregate), Automobile Liability ($500,000 per occurrence), and Cyber Liability ($1,000,000 per occurrence). 2. BACKGROUND

CareOregon manages a non-emergent medical transportation program for eligible Medicaid members to travel to medically necessary services as covered under the Medicaid program. CareOregon currently delegates this authority to Ride Connection, Inc., a non-profit transportation organization to support the Network Management. CareOregon and Ride Connection endeavor to create a non-profit ambulance bariatric stretcher program to serve Medicaid members within the three-county Portland metro area receiving services through the “Ride to Care” program. There is insufficient capacity to serve non-emergent ambulance basic life support, advanced life support, specialty care/critical care, stretcher only, secure, and bariatric ambulance service. This inadequacy has direct negative impact on Medicaid member health outcomes. Likewise, some aspects of health system resources are unnecessarily taxed by lack of ambulance capacity.

The company will be established to primarily serve the three-county metro area region: Multnomah, Clackamas, and Washington counties, operating within and outside the City of Portland and within the State of Oregon.

Financial considerations (including funding methods):

o Startup costs associated with starting a non-profit ambulance company to include licenses, insurance, etc.

o Assets - vehicles and equipment needed, including bariatric equipment. Spec out varying options. Should include consideration of ongoing costs associated with maintenance costs for specialized assets.

o Cost of hiring, training, and retaining qualified EMT’s.

o Potential funding methods for non-profit startup.

Propose steps, including a process with needed expertise necessary to tackle inherent barriers;

A proposed timeline to address these activities should be included;

An analysis on sustainable business model within the current market.

Feasibility Study Deliverables:

Study should identify and outline a pathway to accomplish the proposed activities;
