About this file

This is a Past Performance Questionnaire template issued by the Mission and Installation Contracting Command (MICC) - Fort Eustis for RFP W9124G25R0001, related to the TRADOC G-2 Operational Environment and Core Functions Support Services requirement. The questionnaire must be completed and returned to the Contracting Officer and Contract Specialist by 10:00 AM EDT on February 25, 2025, or within 3 days of receipt, whichever occurs first.

The questionnaire evaluates contractors on four main performance areas using a five-tier rating system (Exceptional to Unacceptable): effectiveness of staffing and management plan, workforce stability, surge support capability, and cost control effectiveness. It requires detailed rationale for each rating and includes sections for general comments on customer satisfaction and other relevant performance information. The template is designed to gather past performance information from previous customers to assist in the evaluation of offers for this 100% Small Business Set-Aside procurement under NAICS 541990.
