About this file
This is an Attachment D Reference Check Form for RFP #S-63500-00016646, issued by the Oregon Department of Fish & Wildlife (ODFW) through DAS Procurement Services. The RFP seeks a contractor to provide helicopter survey services for big game species across Wallowa, Union, and Baker Counties in Northeast Oregon. The contractor must supply two different helicopter types—an R44 and R66 or equivalent models—along with an experienced pilot, fuel truck, and licensed driver to transport ODFW employees during wildlife surveys.
Proposers are required to submit three completed reference check forms from professional references, with at least two references being non-ODFW sources from different firms or clients, and one reference from previous work that may be ODFW-related. The evaluation process assigns a total of 15 points for references, with each reference rated on a scale of 0 to 5 across six criteria: overall quality of services and relationship, safety performance, customer service responsiveness, pilot responsiveness during survey periods, experience in remote wildlife surveys, and likelihood of continued future use. The Agency will make up to three contact attempts for each reference, with proposers receiving zero points for any reference that cannot be successfully contacted.
