Volume 5 – Past Performance

Offeror: ??????

Past Performance: Offerors shall provide three references for whom the Offeror has provided recent (work performed within the last 3 years) and relevant (work very similar to size and scope that is requested here). Offerors may submit information on key personnel, major subcontractors, work performed as part of a team or joint venture, and other previous reincarnation of its current organization. Offerors may also provide information on problems encountered on the identified contracts and a brief explanation of the corrective actions taken. For each reference the Offeror shall provide the following information: agency/business name, a contact name, telephone number, e-mail address, project title, date the services were performed, type of contract (i.e. fixed priced, labor hour, time-and-material) contract value, including options, a brief description of the work being done under the contract, identification of whether or not your company was the prime Contractor or subcontractor, identify major subcontractors, team partners, and their work responsibilities. The Government will also obtain recent and relevant past performance information available from other sources not identified by the Offeror (i.e. CPARS).
