About this file

This is an Addendum #1 to a Request for Proposal issued by the State of Iowa Department of Administrative Services for professional design services related to the Reynolds Building North Annex Roof Replacement project at the Independence Mental Health Institute. The addendum, dated February 29, 2024, clarifies key project coordination procedures and does not change the original proposal due date of March 6, 2024. The addendum confirms that a hazardous materials survey will be performed by an environmental consultant and can be coordinated with the consultant's kickoff meeting and initial field evaluation to verify roof assembly composition simultaneously. Additionally, the addendum confirms that DAS or the Construction Manager will contract with a roofing contractor to perform core cuts and patch them to ensure watertight conditions following the hazardous materials survey. The addendum also confirms that facility staff have a ladder available for the design consultant to access the lower north roof area during site evaluations.

The addendum supersedes and supplements any conflicting portions of the original Request for Proposal documents. Acknowledging receipt of this addendum in the proposal submission is mandatory, and failure to do so may result in disqualification. The addendum does not address pricing terms, set-asides, incumbents, or funding sources, as these matters are addressed in the primary RFP documents and related contract opportunity materials.
