About this file
This document is an Addendum to a Request for Proposal (RFP) issued by the State of Iowa Department of Administrative Services (DAS) for the IMHI Reynolds Bldg North Annex Roof Replacement project located in Buchanan County, Iowa. The Addendum provides clarification on the proposal schedule, coordination of a hazardous materials survey with the design consultant, and the availability of a ladder for accessing the lower roof area. The project scope involves the removal and replacement of the existing EPDM roofing, insulation, gutters, and downspouts on the Reynolds North Annex Building at the Independence Mental Health Institute (IMHI). The proposal due date does not change, and the design consultant awarded the project will coordinate with the environmental consultant and a roofing contractor to verify the roof assembly composition and patch any core cuts.
The Addendum does not provide any details on pricing terms, set asides, incumbents, funding requirements or sources, or contract term and renewal options. It focuses solely on clarifying logistical details related to the hazardous materials survey and roof access for the design consultant.
