About this file

This document is an RFP Submission Checklist designed to assist applicants in preparing complete and timely proposals for Professional Services RFP opportunities with the City of Philadelphia and related agencies in Pennsylvania at or above the formal RFP threshold. The checklist serves as a compliance tool to ensure applicants address all mandatory requirements outlined in the RFP, including demonstration of how the applicant meets the Scope of Work requirements detailed in Section II, submission of a Cost Proposal consistent with specified requirements, and adherence to overall format and content standards described in Section III of the RFP. Applicants are required to review the entire RFP and Contract Attachments, including Appendix A (General Provisions), and must raise any objections to these provisions within their proposal submission, as objections raised after submission will not be considered.

The checklist confirms applicants must submit the completed proposal electronically through eContractPhilly before the stated deadline under the correct opportunity number. Required supporting documentation includes Appendix C (City of Philadelphia Tax and Regulatory Status and Clearance Statement), a Statement of Financial Capacity accompanied by audited or unaudited financial statements, bankruptcy disclosure for the past five years, and IRS Form 990 for nonprofit organizations. Applicants must also disclose all information required under Philadelphia Code Chapter 17-1400, including local political contributions, through their eContractPhilly account. Optional submissions include Appendix D (Local Business Entity or Local Impact Certification) and Appendix E (LGBTQ Applicant Opportunity Data). The application must be signed and submitted by an authorized signatory, and the applicant's registered name and Tax Identification Number must match across the eContractPhilly profile, Appendix C, and W-9 form.
