About this file
This document is a Request for Proposals (RFP) Submission Checklist created by the City of Philadelphia to assist applicants in preparing complete and responsive proposals for Professional Services RFP opportunities at or above the formal RFP threshold. The checklist serves as a verification tool for applicants to confirm they have met all submission requirements prior to the stated deadline. Key submission requirements include demonstrating how the applicant meets the Scope of Work requirements outlined in Section II of the RFP, submitting a Cost Proposal consistent with specified requirements, adhering to all format and content requirements detailed in Section III, and reviewing the entire RFP and Contract Attachments including Appendix A - General Provisions. Applicants must disclose any objections to the General Provisions within their proposal, as objections are not favored and may be weighed negatively during evaluation.
The checklist mandates submission of Appendix C: City of Philadelphia Tax and Regulatory Status and Clearance Statement and a Statement of Financial Capacity along with supporting documentation such as audited or unaudited financial statements, bankruptcy disclosure forms, or IRS Form 990 for non-profit organizations. All required disclosures under Chapter 17-1400 of the Philadelphia Code, including local political contributions, must be completed through the applicant's eContractPhilly account. Proposals must be submitted electronically through eContractPhilly under the correct opportunity number, signed by an authorized signatory using the "sign and submit" button, and the applicant's registered name and Tax Identification Number must match consistently across the eContractPhilly Applicant Profile, Appendix C, and W-9 form. Optional submissions include Appendix D: Local Business Entity or Local Impact Certification and Appendix E: LGBTQ Applicant Opportunity Data.
