About this file

This is an RFP Submission Checklist for the City of Philadelphia, designed to assist applicants in submitting complete and timely proposals for Professional Services RFP opportunities at or above the formal RFP threshold. The checklist addresses the Division of Juvenile Justice Services (JJS) within the Department of Human Services, which is seeking qualified applicants to provide comprehensive vocational training and career development services. Applicants must ensure their proposals are submitted before the stated deadline, explain how they meet the requirements outlined in Section II "Scope of Work," include a compliant Cost Proposal, and adhere to all format and content requirements specified in Section III. All applicants are required to review the entire RFP and Contract Attachments, including Appendix A - General Provisions, and must raise any objections to these provisions within their proposals.

Submission requirements mandate that applicants provide the City of Philadelphia Tax and Regulatory Status and Clearance Statement (Appendix C), a Statement of Financial Capacity accompanied by recent audited or unaudited financial statements, disclosure of any bankruptcy filings within the past five years, and IRS Form 990 for non-profit organizations. Applicants must disclose all information required under Chapter 17-1400 of the Philadelphia Code, including local political contributions, through their eContractPhilly account. Optional submissions include Appendix D: Local Business Entity or Local Impact Certification and Appendix E: LGBTQ Applicant Opportunity Data. All proposals must be submitted electronically through eContractPhilly under the correct opportunity number, signed and submitted by an authorized signatory, with matching name and Tax Identification Number across the applicant profile, Appendix C, and W-9 form.
