About this file

This is an RFP Submission Checklist developed by the City of Philadelphia Department of Recreation for professional services opportunities at or above the formal RFP threshold. The checklist serves as a compliance tool to ensure applicants submit complete, accurate, and timely proposals. Key submission requirements include meeting the stated deadline, demonstrating how the applicant meets the scope of work requirements outlined in Section II of the RFP, providing a cost proposal consistent with specified requirements, adhering to format and content standards in Section III, and reviewing all contract attachments including the General Provisions in Appendix A. Applicants must electronically submit their proposals through eContractPhilly under the correct opportunity number and sign the submission using the designated "sign and submit" button at the conclusion of the process.

Required documentation includes Appendix C (City of Philadelphia Tax and Regulatory Status and Clearance Statement), a Statement of Financial Capacity accompanied by either audited or unaudited financial statements, bankruptcy disclosure information from the past five years, or IRS Form 990 for nonprofit organizations. Applicants must disclose all information required under Philadelphia Code Chapter 17-1400, including local political contributions, through their eContractPhilly account. Optional appendices include Appendix D for Local Business Entity or Local Impact Certification and Appendix E for LGBTQ Applicant Opportunity Data. Critical verification points include confirming that the registered applicant name and Tax Identification Number on eContractPhilly match those on the application and W-9 form, and that the individual signing the proposal is an authorized signatory for the applicant organization.
