About this file

This is a Request for Proposals (RFP) submission checklist developed by the City of Philadelphia for professional services opportunities at or above the formal RFP threshold. The checklist serves as a compliance tool to ensure applicants submit complete, accurate, and timely proposals. Key submission requirements include demonstrating how the applicant meets the scope of work requirements outlined in Section II of the RFP, providing a cost proposal consistent with specified requirements, adhering to all format and content standards in Section III, and reviewing the entire RFP and contract attachments including Appendix A (General Provisions). Applicants must submit the proposal electronically through eContractPhilly before the stated deadline under the correct opportunity number and must electronically sign and submit their application through the platform's designated process.

The checklist requires submission of Appendix C (City of Philadelphia Tax and Regulatory Status and Clearance Statement), a Statement of Financial Capacity, and supporting financial documentation such as audited or unaudited financial statements, bankruptcy disclosures covering the past five years, or IRS Form 990 for non-profit organizations. Applicants must disclose all information required under Chapter 17-1400 of the Philadelphia Code, including local political contributions, through their eContractPhilly account. Optional submissions include Appendix D (Local Business Entity or Local Impact Certification) and Appendix E (LGBTQ Applicant Opportunity Data). Critical administrative requirements include ensuring the applicant's registered name and Tax Identification Number on eContractPhilly match those on the application and W-9 form, and confirming that the individual submitting the application is an authorized signatory for the applicant organization.
