About this file

This is an RFP Submission Checklist document for the City of Philadelphia's Department of Human Services related to Professional Services contracting opportunities. The checklist serves as a compliance tool to ensure applicants submit complete, accurate, and timely proposals for formal RFP opportunities. The document does not specify a particular response deadline, project scope, or contract term, as it functions as a general submission guideline rather than a specific solicitation notice.

The checklist requires applicants to demonstrate compliance with multiple submission requirements, including adherence to the Scope of Work outlined in Section II of the RFP, submission of a compliant Cost Proposal, compliance with format and content standards specified in Section III, and electronic submission through eContractPhilly by the stated deadline. Applicants must submit financial documentation including audited or unaudited financial statements, bankruptcy disclosures, and IRS Form 990 (for non-profits), along with Appendix C (Tax and Regulatory Status and Clearance Statement) and any applicable optional certifications such as Local Business Entity or LGBTQ Applicant Opportunity designations. The checklist emphasizes that all information required under Philadelphia Code Chapter 17-1400, including local political contributions, must be disclosed electronically, and that proposals must be signed and submitted by an authorized signatory with matching name and Tax Identification Number across all registration documents, applicant profiles, and supporting forms.
