About this file

This is an LGBTQ Applicant Opportunity Data Form used by the City of Philadelphia as part of its commitment to diversity, equity, and inclusion in procurement processes. The form is designed to collect information from applicants regarding LGBTQ business ownership and certification status for contracting opportunities with the City. The document serves as a data collection tool to identify companies beneficially owned and controlled by lesbian, gay, bisexual, transgender, and queer persons seeking to do business with Philadelphia and to facilitate the inclusion of LGBTQ-certified businesses in the City's vendor database.

The form requests applicants to provide their business name, bid or opportunity number, and answer four key questions regarding LGBTQ ownership and certification: whether the applicant's business is beneficially owned and controlled by LGBTQ individuals; whether the business holds current certification as an LGBTBE through the National LGBT Chamber of Commerce or its Philadelphia affiliate, the Independence Business Alliance; whether the applicant holds additional third-party certifications such as MBE, WBE, DSBE, or DBE status; and whether any subcontractors are certified as LGBTBE. Response to the form is entirely voluntary, and by submitting it, applicants agree that the City may use the submitted data for other projects and potentially make it publicly available.
