About this file

This is a Reference Check Form attachment for Request for Proposals S-63500-00013532 issued by the Oregon Department of Fish and Wildlife through an intermediate procurement process. The RFP seeks to acquire GPS satellite collars designed for monitoring bighorn sheep populations. Offerors are required to submit three completed Reference Check Forms as part of their proposal evaluation, with one form submitted for each reference. The agency will make up to three attempts to contact each provided reference, and failure to reach a reference will result in a zero score for that reference. Proposals are due by May 15, 2025.

The Reference Check Form evaluates offerors across multiple performance dimensions rated on a scale of 0-10, including overall quality of services and business relationship, adherence to proposed timelines, responsiveness to customer service issues, and likelihood of future engagement. The form includes specialized technical questions specific to GPS satellite collar performance, requesting data on the percentage of collars that experienced battery failure prior to minimum estimated battery life, collar failures due to firmware or software issues, and the percentage of collars that incorrectly identified animal mortality. References must be identified with contact name, telephone number, and email address to facilitate agency outreach and evaluation.
